- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for quotes as no further written solicitation will not be issued. The provisions and clauses in this RFQ are those in effect through FAC 2005-12. This procurement is set-aside for small business. The NAICS Code is 325188. The Regional Contracting Office, Marine Corps Base Hawaii intends to negotiate a Firm-Fixed contract for Sodium Fluoride for Potable Water. The estimated annual requirement is 200 fifty pound bags of 20/40 mesh sodium fluoride to be delivered as needed. Vendors must be registered in the Central Contracting Registration database to be eligible for a Department of Defense contract award. Firms submitting quotes are encouraged to use the Standard Form 1449, Solicitation/Contract/Order for Commercial Items form fou! nd at http://www.gsa.gov/forms/pdf_file and submit a quotation. All qualified responsible business sources may submit a quotation, which shall be considered by the agency. Firms submitting quotes shall provide the information required in FAR 52.212-3 Offeror Representations and Certifications--Commercial Items. If the end product(s) quoted is other than domestic end product(s) as defined in the clause entitled "Buy American Act--Supplies", the offer or shall state and list the country of origin, DFAR 252.212-7000--Offer or Representations and Certifications-Commercial Items (JUN 2005), and DFAR 252.225-7035--Buy American Act-North American Free Trade Agreement Implementation Act-Balance of Payments Program Certificate (JAN 2005)-Alternate I (JAN 2005).
