Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA-245-09-RQ-0037 is issued as a request for quote (RFQ). This RFQ will result in an Indefinite Delivery - Indefinite Quantity contract award. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-27. Only written requests received directly from the requester are acceptable. If requesting by FAX, you may send your request to 304-264-3997, ATTN: Kathryn Leatherman. The North American Industry Classification System (NAICS) number is 325120 and the business size standard is 1000. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON EACH REQUEST. Service disabled veterans and veteran owned small businesses are encouraged to submit offers. The proposed contract listed here is Unrestricted. Item #'s 0002 - 0005 are option years and not presently funded. FOB destination delivery date of 9 December 2008 for first fill with a monthly fill schedule to the VA Medical Center, 510 Butler Ave., Martinsburg, WV 25405. This is a newly installed above-ground tank. There is no historical data and reserve cylinders are available. Task orders for each required fill will be issued for payment by credit card or electronic funds transfer. Contractor will abide by all Medical Center Policies during performance under this contract. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, the Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: 1) Price. Price will be evaluated to determine its fairness, completeness, and reasonableness as it relates to the items offered. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registration has been completed. If ORCA registration has been completed please note in your submission package. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 1, 3, 7, 15, 16, 17, 18, 19, 20, 21, 22, 28, 31 and 37. 52.216-1 Contract Type; 52.217-8 Option to Extend Services; 52.217-9 Option to Extend the Term of the Contract - Performance shall not exceed 5 years, 6 months; 52.228-5, Insurance -Work on a Government Installation; 52.232-19 Availability of Funds for the Next Fiscal Year - Funds are not presently available beyond 30 September 09; 852.216-70 Estimated Quantities; Electronic Funds: FAR 52.252-2, Clauses Incorporated by References (Feb 1998).
