- VA-261-09-RP-0046 Due: December 15, 2008 Request for Proposal (RFP) - Repair and Preventative Maintenance of the Acudose Rx Cabinet and Auxiliary unit at the San Francisco VA Medical Center. The Department of Veterans Affairs intends to award a contract for the aforementioned service, acquisition to be accomplished by the VISN 21 Consolidated Contract Activity. Contractor to provide all labor, supplies, management and general expertise necessary to fulfill requirements. NAICS Code of 811310, size standard of $6.5 million applies. Product Service code of S216 applies. The actual solicitation should be posted on or about December 1, 2008. This is expected to be a five year contract. The intent is for sole source award to McKesson since only they have the propriety technical knowledge to perform this maintenance. Telephone requests will NOT be accepted. Electronic solicitation may be accessed at no cost at www.fedbizopps.gov. Any amendment issued against this solicitation will also be available from the same URL addresses. Contractor is responsible for submitting all documents required in accordance with the solicitation, including any amendments which will be posted on this website. Solicitation is also subject to availability of funding. Proposals shall be submitted on an all or nothing basis. Partial proposals will be rejected. Proposal shall be valid for 60 days from offer due date. The following provisions and clauses apply as appropriate plus any addenda:This solicitation and incorporated provisions and clauses are those in effect through FAC 2005-29. Offeror shall submit all information with offer as required in 52.212-1; a completed copy of provisions 52.212-3 Offeror Representations and Certifications. Offeror may obtain copies of the referenced provisions and clauses at http://arnet.gov/far and http://www.va.gov/oa&mm/vaar/ or by sending an email to the Contracting Officer at bill.ulibarri@va.gov. The Contracting Officer may add any additional contract requirements or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the Contracting Officer to necessary for this acquisition and consistent with customary commercial practices.
