Description
Provide Urgent Laboratory Services for beneficiaries of the VA Salt Lake City Health Care System for patients who must be monitored that are taking Coumadin in the Idaho Falls, Idaho area that are assigned to the Pocatello, Idaho CBOC. The contractor shall provide all necessary labor, equipment, materials and facilities to provide laboratories of an urgent nature. Patients that are drawn by noon shall be faxed the same day to the Pocatello CBOC. Patients that are drawn from 1:00 PM to 4:30 PM shall be faxed the following morning before noon. Services for urgent laboratory testing shall include Prothrombin Time/International Normalized Ratio (CPT Code 85610), and Complete Blood Count (CPT Code 85025) with an estimated volume of 50 to 75 laboratory tests per month. Only fully licensed and accredited laboratories actively engaged in providing the specific services and laboratory testing outlined will be considered. The contractor shall have all licenses, permits accreditation and certificates required by law. The reference laboratory must be licensed and accredited by the College of American Pathologists (CAP) and/or other state regulatory agencies as mandated by federal state statues. In addition the laboratory must be certified as meeting the requirements of the Department of Health and Human Services Clinical Laboratory Improvement Act of 1988. Policies and procedures shall comply with Health Insurance Portability and Accountability Act (HI)PPA) The contract will run from the Date of Award through September 30, 2009 with four one-year options to extend the term of the contract. The contract is an IDIQ contract. The contract will utilize simplified acquisition procedures in accordance with Commercial Items Part 12, and in accordance with Part 15, contracting by Negotiation. The Issue Date is O/A December 11, 2008. The BOD is O/A December 29, 2008.
