Description
NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention (CDC), the National Institute for Occupational Safety and Health, NIOSH, Morgantown, WV, Division of Safety Research, Protective Technology Branch hereby announces its intent to issue a purchase order to Cyberware Laboratory, Inc. for a Dual View Head & Face Color 3D Scanner (PX). Cyberware will supply the complete Dual View Head & Face Color 3D Scanner (PX) system, including the special Dual View PX scan-head configuration, PX overhead motion system, USB power supply, cabling, online manuals, integral system software and services. Integral system software includes node-locked licenses for CyScan and PLYEdit+. Cyberware will also conduct on-site installation and training, provide one-year return-to-factory warranty, and provide email/telephone based technical support and software updates. In addition, Cyberware will supply a single node-locked license for CySlice Software as add-on software to the system. The NIOSH Protective Technology Branch uses a Head and Face Scanner as a valuable tool to conduct research in engineering anthropometry to resolve issues on protective equipment design. NIOSH has been conducting anthropometric research with the existing 3D scanner for over ten years. NIOSH has a need to work with this company to reintegrate hardware as well as software into an upgraded digital system to further our research for occupational safety. This project is supported by justification for other than full and open competition to Cyberware Laboratory, Inc. under authority of 41 U.S.C.253(c)(1). Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. Please forward the capability statement and pricing information to Rebecca Mullenax, Purchasing Agent, Reference 000HCCJC-2009-65260, CDC NIOSH, 1095 Willowdale Road, Morgantown, WV 26505, or e-mail to RMULLENAX@CDC.GOV or FAX to 304-285-6083. Phone 304-285-5880. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all capabilities statements and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with the Cyberware Laboratory Inc., 2110 Del Monte Ave, Monterey, CA 93940-3712 as the only source and a purchase order will be issued without any additional notices being posted.
