- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is IHS-SOL-0900057 for Blood Products and Immunohematologic services. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. The NAIC code is 621991 and the small business size standard is $10M. Blood Product and Immunohematologic services Period of performance: Base Year January 1, 2009 through December 31, 2009; plus four option years. Evaluation will be based on products, technical service, pickup and delivery services and past performance Products is 40 percent; ability to supply blood products routinely and on an emergency basis and ability to supply leukocyte reduced red blood cells, fresh frozen plasma and platelets Technical service is 30 percent; ability to provide technical services for antibody identification, unit screening for particular antigens, antigen typing, comparability testing and consultation services during hospital normal working hours and off hours. Pickup delivery service is 20 percent; ability to provide regular pickup and delivery services for blood products; with STAT services as needed. Past Performance is 10 percent, ability to provide documentation of past performance of satisfactory service performed in a federal facility. Offeror shall provide past performance references with name, contact name, period of performance and telephone number. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. PURPOSE: This is a non-personal services contract for the Claremore Indian Hospital for the time period beginning January 1, 2009 through December 31, 2009, with four pre-priced option years. Background. The Claremore Indian Health Service Hospital is located in Claremore, Oklahoma and serves the Indian population of most of northeastern Oklahoma. The hospital is a 50-bed JCAHO accredited facility which includes an Intensive Care Unit, Obstetrics Unit, Surgery and Same-day Surgery Units, and a General Medical Unit. In addition to the inpatient facilities, this hospital also serves an average of four hundred outpatients daily Monday through Friday in the Medical Clinic, Walk-in Clinic, OB-GYN Clinic, Diabetes Clinic, Surgery Clinic, Pediatrics Clinic, and several other specialty clinics. The Emergency Room is operational 24 hours per day, seven days per week. The Claremore IHS Hospital Laboratory is a College of American Pathologist and CLIA accredited laboratory. It is staffed 24 hours per day, seven days per week and must provide blood products and services for the patient population as prescribed by the Healthcare Provider. The Claremore Laboratory does not have sufficient staffing, equipment, or space to provide these particular services. There are no efforts being made that will duplicate this service. Upon award of a contract all open market purchases will be canceled. Independently, and not as an agent of the Government, the Contractor shall provide blood products and immunohematologic services. The Contractor shall provide the facilities, equipment, personnel, materials and supplies except as may be otherwise provided herein by the Government, Products to be provided: Leukocyte Reduced Red Blood Cells, Fresh Frozen Plasma, Platelets. Technical Services to be provided: The Contractor shall provide professional immunohematologic clinical laboratory testing to include: Antibody identification (including ABO/Rh, antibody screen, complete red cell antigen phenotype, comprehensive DAT and red cell panel), Unit screening for particular antigens, Antigen typing, Compatibility testing to be provided with a turn around time of 4 hours for routines and not to exceed 8 hours for nonroutine or unusual blood requirements. Reports for immunohematologic testing shall be provided by fax within 12 hours of receiving the sample unless (rare instances) the testing requires a longer time for completion. A typed report shall be provided to the facility in the next scheduled pick-up delivery visit or mailed within 24 hours of testing. Consultation Services shall be provided on an as needed basis to be determined by the Blood Bank medical technologist, a contact person to be made available 24 hours per day, seven days per week. Pick up/Delivery Services: The Contractor shall provide two scheduled pick ups and delivery services per week. The contractor shall provide any special transport containers or transport environment needed for preservation of blood products and specimens during the transport. Lost or Unsatisfactory Specimens: The Contractor shall provide a report of lost or unsatisfactory specimens for every lost, missing, damaged, or otherwise unsatisfactory specimen. The Contractor shall provide STAT pick up and delivery services with a response time of one hour. An Indefinite Delivery/Indefinite Quantity (IDIQ) contract will be awarded and task orders issued against the IDIQ contract. Offerors shall submit unit price for each product listed for the base plus option years. FAR part 17 will be applicable to the awarded contract.
