Haley Veterans Administration Medical Center (JAHVAMC) 13000 Bruce B. Downs Blvd Tampa Fl to include VA Port Richey Outpatient Clinic, and Community Based Outpatient Clinics at Brooksville, Zephryhills, and Lakeland Section 1.0 Description of Services/General Information 1.1 SCOPE 1.1.1 The James A. Haley Veterans Hospital, a division of the United States Government Department of Veterans Affairs (hereinafter called the VA), has a need for specialized medical and technical resources to be furnished by the Contractor. The VA is located in Tampa, Florida and is one of the VAs VISN 8 facilities which include VA-staffed Outpatient Clinics at Port Richey, and Community Based Outpatient Clinics at Brooksville, Zephryhills, and Lakeland. The specialized medical resources required are to provide Radiation Safety Officer Services (not less than.5 FTEE on-site presence as well as appropriate and complete management of the Radiation and Laser Safety Programs. Hours to be worked on site can be flexible.), including On-Call Services in accordance with the requirements of the contract. Contractor shall comply with the following: 1.2 TERM OF CONTRACT 1.2.1 The term of this contract shall be from January 3, 2009 (or date of award, whichever is later) through January 2, 2010. 1.3 Hours of Operation/Response Time/Billing 1.3.1 Provide services 24hrs a day, seven days a week, 365 days a year. 1.3.2 Respond to calls for service within 3 hours of call and be on site. 1.3.4 Invoicing shall be accomplished monthly and include the date when services were performed and the number of hours worked. 2.0 DESCRIPTION/QUALIFICATIONS/SPECIFIC TASKS 2.1. GENERAL STATEMENT OF WORK - Radiation and Laser Safety Officer Services, including On-Call Services 2.1.2 The contractors qualified radiation and laser safety officer shall provide radiation and laser safety officer(s) during the contract period for the performance of services, including on-call services as outlined below. On-call services involve the contractor being available by phone to respond and come to the hospital if necessary either for consultation and/or to evaluate and supervise radiological situations. The primary time of these on-call services will be during non-duty hours of 4:00 PM to 7:30 AM., when VA staff is not available. In reality, although only.5 FTEE is cited, the anticipated commitment in time is expected to require a full time on-site program management in order to meet regulatory compliance conditions. The contractor shall comply with conditions of 10 CFR 35.24 Authority and Responsibilities for the Radiation Protection Program and all conditions for the proper maintenance of both safety programs. The contractor is to be on site not less than 20 hours per week prorated. The.5 FTEE commitment shall be during normal business hours. Normal business hours are Monday through Friday 7:30 a.m. to 4:00 p.m. excluding the Friday after Thanksgiving and all Federal holidays. 2.1.3 Contractor will attend VHA training and quality conferences as required by the medical center director. Travel days will be considered days on service. Contractor will be reimbursed for travel cost, subject to prior approval. The contractor will be paid by the hour, without additional compensation for more than 8 hours per day. 2.2 Qualification Requirements 2.2.1 Meets the requirements as stated in Energy; 10 CFR 35.50. 2.2.2 Meets the requirements relative to the experience needed to administer a Type A Broad Scope License Permit for the possession and use of radioactive materials for human diagnostic, therapeutic, human, animal, and in-vitro research procedures. Ability to evaluate and regulate the medical research use of diagnostic radiography and therapeutic LINAC machine generated radiation. 2.2.3 Meets ANSI recommendations and OSHA requirements as a Laser Safety Officer for the medical application of type III and IV laser devices. 2.2.4 State Licensed as a Medical Nuclear Radiological Physicist or equivalent. 2.2.5 Acceptable to the Director of the National Health Physics Program to function as the Radiation Safety Officer of the Type A License Permit operating under the Nuclear Regulatory Commission issued V.A. Master Materials License. 2.2.6 The following shall be provided as proof of meeting the aforementioned qualifications required in section 2.2 of this SOW: a. Copy of appropriate health physics state licensure is required to be provided with quote submission. b. Evidence of eligibility meeting NRC Type A broad scope license permit, e.g. copy of license showing contractors name so listed which demonstrates conformance to CFR 10 Parts 1-199 and all conditions as specified in the facility license permit. 2.3 Specific Tasks 2.3.1 Conducts patient and employee radiation exposure monitoring of the various patient care activities of SCI, G.I., Pulmonary, Surgery, Radiation Oncology, Research, Cardiology, Nuclear Medicine, Radiology, Anesthesiology and Ophthalmology at the Tampa VA sites. Provides information feedback to the health care provider relative to each patient receiving ionizing radiation from fluoroscopic devices offering the reasonable possibility of radiation injury or violation of the conditions of regulatory compliance. Initiates as necessary, the evaluation and treatment of patients affected by radiation with upper management involvement. 2.3.1 Conducts the Facility Occupational and environmental radiation exposure and dosimetry program. 2.3.2 Functions as the Executive Agent of the Radiation Safety Committee, and is named on the License Permit as the Facility Radiation Safety Officer with associated regulatory responsibilities. 2.3.3 Maintains a waste storage and disposal program for all radioactive waste materials. Utilizes all methods of disposal to include decay in storage, sewer system disposal, atmosphere release, and burial. for liquids, solids, biological materials, animal carcasses, patient administration devices, shielding containers and unwanted equipment containing sealed sources and irradiating parts. 2.3.4 Assimilates the agenda items to be presented to the RSC. Presents to the Committee all issues associated with the possession, use, and disposal of radiation and radioactive materials. 2.3.5 Functions as the Facility Laser Safety Officer. Presents to the Facility Laser Safety Committee all issues associated with the possession and use of medical laser devices. 2.3.6 Reviews all program applications and request for Principal Investigator status and Authorized User status for each specific requested activity for credibility, need, application of use, competency of personnel involved, investigates history of use at this and other facilities, requirements for education, training, and experience for the specifics of the request. 2.3.7 Develops and implements Standard Operating Procedures (SOPs) for the application of radioactive materials and laser devices. 2.3.8 The incumbent will evaluate, monitor, and insure that restricted radiation areas are appropriately restricted from inappropriate use. Conducts routine area monitoring procedures. Radiation exposure into unrestricted areas will be evaluated, monitored with conformance for allowable exposure as indicated by Code and Law, and good practice and sound evaluative judgment. VA, and other Federal regulations and accreditation standards, relative to safe environment of care conditions are to be applied. 2.3.9 Conducts education sessions with all individuals having access to radiation restricted areas, and, or are, the users of radioactive materials. Training may also be conducted as necessary with users of machine generating radiation as appropriate. The training presented is customized and specific to the needs of the individual(s) 2.3.10 Conducts internal and external radiation dosimetry determinations for patients, employees, and members of the general public as associated with the application and use of radio-chemicals, radiopharmaceuticals, implants, and exposure from diagnostic and therapeutic equipment generated radiation as needed. 2.3.11 Application of the methods used to determine the shielding requirements for radioactive materials, diagnostic radiological equipment, sealed source materials, and therapeutic Linear Accelerators. This includes installation oversight and shielding evaluation. 2.3.12 Prepares the necessary regulatory compliant reports in the event of radiation administration errors, medical events, and mis-administrations for diagnostic and therapeutic radiopharmaceuticals and radio-chemicals as well as events-of-error in Radiation Oncology. This function includes medical laser device events. Edits and submits for approval, the Radiation Safety Manual, the Laser SOP Manual, and the ALARA Program. 2.3.13 Directs the decontamination process of all radioactive material contaminations. Determines the Xenon-133 clearance rates for all areas of use. Evaluates on an ad-lib basis, all users of radioactive materials for safe practice with corrective instruction as appropriate. 2.3.14 Reviews research applications and protocols as assigned by the Research and Development Committee. 2.3.15 Maintains records and documents in the Office of Radiation Safety in a regulatory compliant manner. 2.3.16 Prepares and presents to Upper Management, an annual report of the status of the Radiation Safety Program with interim reports as radiological events occur. 2.3.17 Represents Upper Management with stop-work authority in events of significant radiological unsafe practice conditions. 2.3.18 Is present to conduct planned and unannounced accreditation and regulatory on-site inspections. 3.0 Government Furnished Property (GFP) and Services: 3.1 General office furniture and equipment, including computer will be provided by the hospital along with personal dosimitry service.
