Description

The Billings Area Indian Health Service is requesting proposals in response to Request For Quotation (RFQ)-10-09-016-TD to provide preventive maintenance and repair for the Siemens Mammomat Nova 3000 at the Fort Belknap Service Unit, PHS Indian Hospital, Harlem, Montana. The contract shall be awarded pursuant to Federal Acquisition Regulation (FAR) Subpart 13 Simplified Acquisition Procedures, and FAR Subpart 37.4, Nonpersonal Healthcare Services (10 U.S.C. 2304 and 41 U.S.C. 253), FAR Subpart 13.5, Test Program for Certain Commercial Items, and FAR Subpart 37.6, Performance-Based Contracts. This request is being solicited on an unrestricted basis. The contract will be awarded for a base year (tentative start date is January 1, 2009 through September 30, 2009) with four 12-month options. Prospective contractors may access the solicitation from the Federal Business Opportunities website once uploaded or request a copy of the solicitation by contacting the Indian Health Service Contracting Office at (406) 247-7293, by faxing a request to (406) 247-7108, or by e-mailing tammy.dennert@ihs.gov. The tentative issue date of the solicitation is December 11, 2008, with a response date of December 19, 2008.
