- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W90ED583050100 is issued as a Request for Quote (RFQ). This request for Quote is due by 29 December 08, 12:00 (pacific time) The RFQ will result in a firm fixed price contract. This solicitation is 100% set aside for small business. The North American Industry Classification System (NAICS) code for this acquisition is 621340. This acquisition is under Fede-ral Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 13.5, Test Program for Certain Commercial Items.. This requirement is for: SERVICES NON-PERSONAL: Furnish all labor, tools, equipment, material, transportation and supervision to: Yakima Readiness Center, 2501 Airport Lane, Clinn 0001 Hearing Exams 50ea. $______________. SOW for Hearing Exams 9-10 Jan 2009. 1.Need Mobile Audiology Services at the Yakima Readiness Center in Yakima Wa for approximately 50 soldiers. This baseline exam will conform to AR-501 standards and results posted on a DD form 2216E. 2.MUST HAVE DOEHRS certified audiology and capability to update audiogram in the DOEHRS database. 3.MUST HAVE MOBILE VAN capability for this event. Contractor should NOT assume there will be adequate power connection with local buildings and must bring their own generator. 4.Anticipated event times are Noon-1800 on Friday 9 Jan 09 and 0800-1800 on Sat 10 Jan 09. This does not include set up and tear down time. Expect to be able to run continuously throughout this time period. 5.Contractors must provide sufficient personnel and equipment to accomplish the request number in the time specified. 6.Documentation of all exams must be completed and a copy placed in the soldiers medical record same day of event. 7.Need a list of Soldiers by name and Social Security Number with their hearing rating listed (H1,H2, or H3) provided to the Deputy State Surgeon within 24 hours of the events completion. This list can be faxed to 253-512-7533. 8.POC is LTC Munsey at 253-405-5757 or SFC Rey Javier at 253-405-5536. 9.POC at the unit is LT Stephen McPherson at 509-469-4605.
