Description
The United States Property and Fiscal Office for New York (USPFO-NY) proposes to procure Dental Screening Services and associated panographic dental radiographs for Soldiers participating in an annual readiness event conducted at Cortlandt Manor, NY on 9-11 January 2009. The contractor is required to provide all material (except that identified in the in the Statement of Work (SOW) as Government Furnished) labor, supervision and means required to perform the following: The objective is to conduct and complete the readiness requirement of the Annual Dental Screening Exam on Soldiers of the 42nd ID STB utilizing a mobile dental facility. The requirements include: 1. Clinical evaluation of the oral cavity by a licensed dentist employing visual clinical exam supported by bitewing dental radiographs. Panographic dental radiographs must represent the Soldier’s current dental condition. Panographs are to be taken if there is no panograph in the record, if an existing panograph is greater than 10 years old, or if the Soldier has had major dental work since last panograph. Also the dentist may order a panograph if deemed clinically necessary to complete a proper dental exam. Radiographs will be taken by dental technicians. Existing adequate panographs that are analog images will be scanned electronically and uploaded into DENCLASS by the contractor. 2. Each exam will be entered into the DENCLASS reporting system within 24 hours of examination by ASM Research (ASMR) trained personnel, annotated on the Standard Form (SF) 603 (Health Record – Dental) and on the SF 5570 (Health Questionnaire for Dental Treatment) of the Soldier’s dental record. 3. Personnel performing the dental screening examination have a moral and ethical obligation to inform the Soldier if they observe or are apprised of any sign or symptom for which the Soldier should obtain further evaluation or care. 4. A dental screening examination identifies deficiencies but does NOT include the dental treatment required to correct those deficiencies. GOVERNMENT-FURNISHED PROPERTY AND INFORMATION: Operating space will be provided for Occupational Safety and Health Administration (OSHA) compliant Mobile Dental Practices. CONTRACTOR FURNISHED ITEM: Contractors will provide all items and licensed personnel needed to complete the Dental Readiness requirements, any employee benefits, Vacation days, Paid Holidays and Sick Leave plan. The Mobile Dental Practices must: (1) provide x-ray areas with lead-lined walls and leaded glass for any patient observation points, for x-ray radiation protection, (2) perform digital bite-wing and panographic x-rays, as needed, for exam completeness and to provide digital x-rays for DENCLASS files, (3) have available emergency medical resources consistent with OSHA standards, such as eye-wash station, distilled water tanks, (4) have separate and walled dental operatories to provide confidentiality for patients (5) provide temperature controlled rooms consistent with OSHA standard, (6) have sterilization equipment in separate sterilization area, biohazard waste receptacles, fully functional standard dental chairs, a designated reception area with fully functional office technology including a LAN with redundant computer systems with printing/copying capabilities, and a ventilation system on-board to minimize air-born pathogens. The contractor’s safety program includes equipment training, occupational safety, health, and fire protection guidance to ensure the protection and health of all work personnel. Contractor shall possess an owned fleet of mobile operatories containing a minimum of three (3) vehicles to accommodate multiple events at the same time. Must be ADA compliant with wheel chair access and an onboard wheel chair lift. Government property will also be protected during the period of time such property is under the control or possession of the contractor. 6. SPECIFIC TASKS: Perform a projected 350 dental examinations, to include digital bitewings; with digital panographic x-rays as indicated, in support of mandatory annual dental screenings during 9-11 January 2009. Dental staff will be prepared to work on Friday, 9 January from 1300 to 1800; Saturday, 10 January, 0700 to 1800; and Sunday, 11 January, 0700 to 1300. These tasks will be performed the Camp Smith Training Site, Cortlandt Manor, NY, 10567. Staffing and equipment levels provided by the contractor must be adequate to complete a minimum average of 24 dental screening exams per hour. At the accepted rate of 8 exams per hour, per dentist, three dentists at a minimium are required for this mission, plus the appropriate number of dental assistants to operate the radiographic stations and assist the dental providers; plus administrative support personnel to include an events manager and two personnel each at check in and check out positions of the dental station area. Three bite wing stations are required. Portable bite wing radiographic equipment is acceptable.
