This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number, VA-249-09-RQ-0064, is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 335110. This procurement is full and open competition. Quotations are due on or before December 30, 2008 by 12:00 PM Central Standard Time to be considered responsive. Item Description: 1. Contractor to supply all parts, labor and equipment to maintain and repair the Parking lot light fixtures located at 1030 Jefferson Ave Memphis, Tennessee 38104. 2. Service to include furnishing and replacing all defective parts, such as lamps, ballasts, refractors, gaskets, damaged wiring from the junction at base of pole to fixture. 3. Contractor to provide one (1) annual cleaning during the year to include washing fixture/refractors inside and out. Cleaning shall be coordinated with the COTR 4. Service calls shall be requested by Electric Shop personnel to Contractor. Contractor shall respond within three to five working days for normal calls and within 24 hours for an emergency calls. 5. Contractor shall provide written documentation of the completed work to Electric Shop Supervisor after each completed service call. 6. Work shall be coordinated to ensure availability to access light poles and to comply with all applicable safety procedures to ensure patient and visitor safety and to prevent possible damage to vehicles parked in the area. 7. During inclement weather with excessive winds and lightning in the immediate area operations deemed unsafe will cease and will not be allowed, to prevent possible injury to contractor, employees and or VA staff as well as patients and or visitors including vehicles. 8. Contractor shall comply with all applicable OSHA rules and regulations during the performance of their duties. 9. The following equipment is to include: A. North Employee Parking; (9 each) 250 watts H.P.S. fixtures and lamps. B. Ambulatory and East Visitors Parking; (43 each) 250 watts H.P.S. fixtures and lamps. C. Bldg. 10: 1101 Poplar Ave; (5 each) 180 watt L.P.S fixtures and lamps. D. Spinal Cord Injury Unit Service lane; 1. (3 each) 70 watts H.P.S. fixtures and lamps. 2. (4 each) 150 watts H.P.S. fixtures and lamps. 3. (6 each) 400 watts H. P. S. fixtures and lamps. E. Walkway Post Lamps; (15 each) 175 watts Mercury Vapor fixtures and lamps. F. West Parking Lot; 1. (29 each) 250 watts H.P.S. fixtures and lamps 2. (6 each) 100 watts H.P.S fixture and lamps G. Clinic Annex Parking; (4 each) 180 watts L.P.S. fixtures and lamps. H. South Employee Parking Lot; (32 each) 400 watts H.P.S. fixtures and lamps Period 1-1-09 to 9-30-09 Option to extend 4 years. Option year 1: 10-1-09 to 9-30-10 Option year 2: 10-1-10 to 9-30-11 Option year 3: 10-1-11 to 9-30-12 Option year 4: 10-1-12 to 9-30-13 Delivery shall be FOB DESTINATION to VA Medical Center, 1030 Jefferson Ave Memphis, TN 38104.
