- The Centers for Disease Control and Prevention will be issuing a Request for Proposal (RFP) to provide program evaluation technical assistance to agencies and organizations that are implementing innovative programs, policies, and organizational structures and processes to prevent health risk behaviors and adverse health outcomes among youth. The NAICS code for this RFP is 541690. The Period of performance will be a five (5) phase contract with a 12-month period of performance for each phase. This requirement is being procured as a full and open competition. The purpose of this acquisition is to provide program evaluation technical assistance to agencies and organizations that are implementing innovative programs, policies, and organizational structures and processes to prevent health risk behaviors and adverse health outcomes among youth. The objectives of this contract are for the contractor, with guidance from DASH staff, to: 1. Provide selected agencies and organizations with information and recommendations that will improve the effectiveness of innovative programs, policies, and organizational structures and processes to prevent health risk behaviors and adverse health outcomes among youth. 2. Provide selected agencies and organizations with their own data to conduct further analyses at their discretion. 3. Provide selected agencies and organizations with measures and data collection systems that they can use after the evaluation is ended to continue to assess their program, policy, or organizational structure or process. 4. Provide the field, when applicable, with information about the effectiveness of selected innovative programs, policies, and organizational structures and processes to prevent health risk behaviors and adverse health outcomes among youth. All deliverables to be disseminated to the programs or the public will comply with Section 508 (29 U.S.C. 794d) standards and requirements of the Rehabilitation Act. The resultant award is expected to be a Cost Plus Fixed Fee Contract. The competitive RFP and all questions and responses related to this procurement will be made available under RFP number 2009-N-10991. This requirement is contingent upon the availability of funds. The RFP is anticipated for release on or about December 23, 2008. Interested parties must respond to the RFP in order to be considered for award of any resultant contract. All responsible sources may submit a bid, proposal, or quotation which shall be considered by the agency. No written solicitation document is available, telephone requests will not be honored and no Offeror's list will be maintained. Notifications will not be sent to the offerors regarding the issuance of the solicitation nor any of its amendments. It is the offerors responsibility to check this website.
