- Combined Synopsis/Solicitation This is a combined synopsis/solicitation for the acquisition of commercial items prepared IAW the format in FAR Subpart 12.6 and as supplemented with additional information included in this notice. The Solicitation number is FA7014-09-R-0004. This announcement constitutes the only solicitation. Offerors are responsible for monitoring this site for the release of any amendments or other pertinent information. This is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-29 and DFARs Change Notice (DCN) 2008-12-18. NAICS is 621399 and the size standard is $7,000,000. This acquisition is unrestricted. The Air Force Medical Operations Agency (AFMOA) is tasked with providing Air Force Health Promotion Operations (HPO) and supplements Air Force (AF) health personnel to develop, promote, and implement AF health promotion (HP) policy, at headquarters (HQ). It also implements the Health Promotions Program (HPP) through Health Educators and Dieticians employed at AF bases throughout the United States. This requirement shall implement AF HP policy through collaboration with site commanders, MAJCOM and Field Operating Agency staff. This requirement shall assist in developing and recommending support to medical staff, targeted education and interventions, worksite and community programs, collaboration and coalition building, health communication, recommending/advocating for policy and environment change to leadership. The HPP is intended to maximize human performance (to include war fighter and workforce capability), as well as minimize preventable health care costs of beneficiaries associated with lifestyle choices to include but not limited to physical activity, nutrition and tobacco use. Air Force District of Washington Medical Support Branch (AFDW/A7KM) Contracting Office is soliciting for the following positions: AF Health Promotion Coordinator for Marketing and Training (HPCMT), AF Health Promotion Program Manager (HPPM), AF Community Nutrition Program Manager (CNPM), Installation Health Promotion Manager (IHPM), Installation Community Dietitian (ICD). The contractor must possess the specific technical skills and knowledge along with operational expertise and experience as described in the Performance Work Statement (PWS). AFDW/A7KM is contemplating a six (6) month Base Period Firm-Fixed Price contract with four (4) One Year Option Periods. Location of required services are CONUS bases and Alaska and Hawaii. Potential offerors are to review the SF1449 (attachment 1) which includes the Performance Work Statement, Instructions to Offerors, Evauation Criteria, applicable provisions and clauses, and pricing tables.
