- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ). This solicitation document and the incorporated FAR provisions and clauses are those in effect through Federal Acquisition Circular 2005-28. This requirement is available for full and open competition - the applicable NAICS code is 339112. The Philadelphia VA Medical Center (PVAMC), 3900 Woodland Avenue, Philadelphia, PA 19104 is soliciting quotations from vendors who can provide some or all of the required interventional supplies for its interventional cardiology program starting in the Spring of 2009. The description of required items stated below is generic. Prospective vendors should provide information about product line(s) relating specifically to interventional cardiology, including any volume/tier/pricing information and consumer/customer advantages of the offered product. Preference will be given in selecting vendors to those vendors willing and able to provide drug eluting stents on a consignment basis. Under the consignment agreement, the contractor will be responsible for stocking products at PVAMC and remaining responsible for such products until the products and purchased and used by PVAMC. Initial contract period will be one year, with two one-year options exercisable by the Government. Pricing should be provided for the base year, and for each of the two option years. Vendors are permitted to provide multiple, alternative offers for different types of equipment that it believes will meet the requirements of PVAMC's interventional cardiology program. Interventional supplies required: Coronary Guide Wires Initial stock of 100 Coronary Wires, exchange length. Please include your item description, part number and pricing for the following: "Straight Guide Wire: extra support, intermediate, standard, floppy "J Tip Guide Wire: extra support, intermediate, standard, floppy "Marker Guide Wire: extra support, intermediate, standard, floppy "J Tip Guide Wire: extra support, intermediate, standard, floppy "Straight hydrophilic Guide Wire: extra support, intermediate, standard, floppy "J Tip Guide Wire: extra support, intermediate, standard, floppy Bare Metal Stents Initial stock of 80, plan to award one vendor for this product: Please include your item description, part number and pricing for the following: Bare metal stents of various sizes ranging from 2.0 to 5.0 in diameter, short, medium and long length. Also include product specifics as to type of metal, stent flexibility, shelf life and clinical advantages of product offering. Drug Eluting Stents Initial stock of 90, plan to award two vendors for this product: Please include your item description, part number and pricing for the following: Drug eluting stents of various sizes ranging from 2.0 to 5.0 in diameter, short, medium and long length. Please include product specifics as to the "drug" eluted, stent flexibility, shelf life, type of metal and clinical advantages of product offering. Guide Catheters Initial stock of 120, plan to award one vendor for this product: Please include your item description, part number and pricing for the following: Guide Catheters, ranging in size from 6 Fr. To 9 Fr and include catheter lengths from 90 to 120 cm in various shapes to accommodate the native coronary system, bypass grafts and specialty catheters used in selective coronary cannulization. Please include product line information as to the lumen diameter, material and clinical advantages of product line offering. Coronary Balloon Catheters Initial stock of 100 balloon catheters, plan to award two vendors for this product. Please include your item description, part number and pricing for the following: Coronary Balloon Catheters, ranging in size balloon size from 1.5 to 5.0 inflated diameter, short, average and long in balloon length. Please include product line offering of compliant and non-compliant balloons, over-the-wire and monorail (rapid exchange). PVAMC will also require product information as to latex content, type and durability of material and any clinical advantages of product line offering. Indeflator Packs Initial stock of 20 packs, plan to award one vendor for this product. Please include the contents of packs and describe the clinical advantages of the product line offering. Please note that the PVAMC is looking for Indeflator Packs to include a torque device, a touhey/Y adaptor, standard sterile medication labels and/or standard, labeled PTCA accessories. Infusion Catheters Initial stock of 10, plan to award one vendor for this product. Please include the size, length and clinical advantages of the product line offering of coronary infusion catheters. State clinical advantages, latex content and product specifics as to size, length and internal lumen diameter. If there are accessories required specific to the catheter/product offering, please include that information as well. Flexible Sheaths Initial stock of 30, plan to award one vendor for this product. Please include the available (adult) sizes (6 to 8 Fr.), medium to long in length meant for the purposes of catheter support (femoral). Include information as to the clinical efficacy of product offering and the material it is made of. Award will be made to the contractor(s) whose proposals are determined to be most advantageous to the Government, price and other factors considered.
