Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The California Air National Guard 129th Rescue Wing intends to establish one or more Blanket Purchase Agreements (BPA) with diagnostic testing laboratories in the Moffett Federal Airfield to satisfy testing requirements for people during Unit Training Assembly(UTA)weekends. The scope of this BPA shall be limited only to the requested diagnostic services for the period of 01 February 2009 through 30 September 2012 if and when requested by the Contracting Officer or his authorized representatives (identified in the call authorization letter). Charges against this BPA will be for requested diagnostic services only. The issuance of individual calls against this Blanket Purchase Agreement will be made in accordance with the Federal Acquisition Regulation parts 6 and 13. The diagnostic testing service provider will perform the following: 1. Pick up processed specimens from the 129 MDS on day of UTA by 1600 hours(4p.m.) 2. Conduct the requested clinical analysis. 3. Deliver results to the 129 MDS within 48 to 72 hours of pick up. 4. Retain specimens for seven days enabling the 129 MDS physician to review results and request repeat or additional tests if required. 5. Furnish all necessary supplies and additional assistance as needed. The diagnostic testing service provider must be: 1. Accredited and certified by both state and local health agencies. 2. Certified and licensed for all approved laboratory services. 3. Available for emergency service during normal workday, upon telephone request from 129 MDS representative, with call-in or fax results available same day. 4. Capable of performing clinical analysis on basic required tests which will consist of but are not limited to the following: Complete Blood Count (CBC), Urinalysis (UA), Glucose, Liver Panel, Humman Immuno Virus (HIV), ABO/RH, G6PD, Sickle Cell Screen, PAP smear, Urine for Benzene, Fecal Occult, and Lipid Panel.
