Description
This is a combined synopsis/solicitation for commercial items prepared inaccordance with the format in Federal Acquisition Regulations document (FAR)Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number FA7000-09-T-0004 is being issued as a Request for Quotation (RFQ) in accordance with FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-29. All applicable provisions and clauses are included in the actual solicitation. This electronic announcement constitutes the only solicitation being issued. This procurement is unrestricted by business size and socio-economic category and competition is full and open. The North American Industry Classification System (NAICS) code is 112511, and the small business size standard is $0.75 million. Women and women owned, service disabled veteran owned, and small disadvantaged businesses are strongly encouraged to submit quotes. The Government intends to issue a firm fixed price contract for the following: The United States Air Force Academy (USAFA), 10 CES/CECN, has a requirement for the annual supply and delivery of 13,700 pounds of whirling disease negative hatchery raised 9 inch to 11 inch rainbow trout for recreational fishing lakes on the US Air Force Academy and Farish Recreation Area in accordance with the attached Statement of Work, one base year plus four option years. The period of performance is from 15 March 2009 through 14 March 2010 with four (4) one year option periods. See the Statement of Work (attached) for specific details. There are ten (10) CLINS. CLINS 0001, 0002, 1001, 1002, 2001, 2002, 3001, 3002, 4001 and 4002: (USAFA lakes) CLINS 0001, 2001, 3001 and 4001; Provide six (6) deliveries each of 1450 pounds (totaling 8700 pounds) of whirling disease negative, 9 inch to 11 inch rainbow trout to be divided between the five (5) fishing lakes on USAFA. Deliveries will occur between March and October, in accordance with the attached Statement of Work. Delivery is to be made FOB destination. (Farish Lakes) CLINS 0002, 1002, 2002, 3002 and 4002; Provide five (5) deliveries each of 1000 pounds (totaling 5000 pounds) of whirling disease negative, 9 inch to 11 inch rainbow trout to be divided between the three (3) fishing lakes at Farish Recreation Area. Deliveries will occur between April and October, in accordance with the attached Statement of Work. Delivery is to be made FOB destination. Please use the attached pricing sheet. Provide a price quote for each of the following CLINS and total price (fill in all blanks): CLIN ITEM DESCRIPTION PRICE PER DELIVERY TOTAL ITEM PRICE Base Year 0001 Provide six (6) deliveries each of 1450 pounds (totaling 8700 pounds) of whirling disease negative, 9 inch to 11 inch, rainbow trout to be divided between the five (5) fishing lakes on USAFA. Deliveries will occur between March and October, in accordance with the attached Statement of Work. Delivery is to be made FOB destination. Period of performance is March 15, 2009 through March 14, 2010. The contractor shall provide a copy of the Fish Health Inspection Report from the hatchery of origin to the Government representative(s) prior to placing any fish in the lakes. 0002 Provide five (5) deliveries each of 1000 pounds (totaling 5000 pounds) of whirling disease negative, 9 inch to 11 inch rainbow trout to be divided between the three (3) fishing lakes at Farish Recreation Area. Period of performance is March 15, 2010 through March 14, 2011. The contractor shall provide a copy of the Fish Health Inspection Report from the hatchery of origin to the Government representative(s) prior to placing any fish in the lakes. 1002 Provide five (5) deliveries each of 1000 pounds (totaling 5000 pounds) of whirling disease negative, 9 inch to 11 inch rainbow trout to be divided between the three (3) fishing lakes at Farish Recreation Area. Period of performance is March 15, 2013 through March 14, 2014. The contractor shall provide a copy of the Fish Health Inspection Report from the hatchery of origin to the Government representative(s) prior to placing any fish in the lakes. Total Price: The provisions at 52.212-2, Evaluation-Commercial Items, apply to this acquisition. The Government will award a firm fixed price supply contract from this solicitation to the responsible Offeror who offers the lowest price. The Government intends to evaluate offers and make award without discussions; however, this does not preclude the Government from having discussions if it is in the best interest of the Government to do so. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with all requirements, that offer will not be acceptable. Provide prices for all CLINS. The clause at 52.237-1, Site Visit (APRIL 1994), as modified, applies to this acquisition. A site visit will be held Tuesday, January 20, 2009 from 9:00 a.m. to 12:00 a.m. Mountain Standard Time. Participants of the site visit will meet in Conference Room A, Building 8110, Industrial Drive, USAF Academy, CO 80840. Submit the names of all attendees (not to exceed four (4) per firm) to Scott Rogers or Carol Mohr no later than Thursday, January 15, 2009, 3:30 PM Mountain Standard Time. This information must be provided in advance in order to ensure adequate seating and government provided transportation for the conference and site visit attendees. Please ensure vehicle proof of insurance and registration are current and bring a government issued photo ID (i.e., state issued drivers license, retired military ID, etc) or you will not be allowed access to the base. Information provided at this conference shall not qualify the terms and conditions of the solicitation and specifications. Terms of the solicitation and specifications remain unchanged unless the solicitation is amended in writing. If an amendment is issued, normal procedures relating to the acknowledgment and receipt of solicitation amendments shall apply. All questions shall be submitted in writing within 24 hours after the site visit ends on January 20, 2009 unless otherwise directed. Please submit questions only by e-mail and be as clear and concise as possible. Reference sections of the contract or Scope of Work to include page and paragraph etc. to ensure an accurate response. A record of the site visit shall be made available to all prospective Offerors on the Federal Business Opportunities web site. The record will include minutes of the site visit, including the sign-in sheet, questions on a non-attribution basis and answers. Questions asked verbally or in other forms will not be addressed. The provisions 52.212-1, Instructions to Offerors, apply to this acquisition. In your quote, please provide: company name, address, telephone number, date of offer, offer number, quote price; Tax ID #; Cage Code and DUNS #. Additionally, your company must be registered in the Central Contractor Registry (CCR) System and your online Representations and Certifications must be completed at the following website: http://orca.bpn.gov/publicsearch.aspx. Offers shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications--Commercial Items, and the 252.212-7000, Offeror Representations and Certifications, with its offer. Quotes must be received NLT 3:30PM MST, Monday, 26 January, 2009, by e-mail at scott.rogers@usafa.af.mil, FAX 719-333-6608, in person, or mail to: Mr. Scott E. Quotations received after this date and time will be considered late and will not be evaluated. The Government will not reimburse any interested parties for any monies spent to provide a response to this combined synopsis/solicitation.
