- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. DESCRIPTION: This requirement is for one (1) year from date of award with four (4) option years. Provide pricing for base year and each option year for the following tests: ABO & Rh Acute Hep Panel BMP8 CBC w/Diff w/o Plt CBC w/Diff/Platelet CBC w/o Diff w/o Plt CBC w/o Diff w/Plt Chain of Custody 2nd Sample Handling Drug Abuse 7+Alc-Unb Chem 26 Chlamydia/GC DNA Probe Cholesterol Cholinesterase, RBC CMP12+8AC CMP14 CMP14+LP CMP14+LP+CBC/Plt CMP14+LP+PSA+UA/m CMP14+LP+RPR+CBC/Plt CMP14+LP+RPR+Sickle+CBC/Plt CMP14+LP+UA/m+CBC G6PD Gynecologic Mono Glucose, Plasma hCG, Qual hCG, Quant Heavy Metals Profile Hematocrit Hemoglobin Hemoglobin Aic Hepatic Function Panel 7 HgB Solubility Iron & IBC Lipid Panel Malarial Smear Microscopic Exam Mono Test Occult Blood Opiates Conf Ova & Parasite Panel Pap Smear (1Slide) Platelet Count PSA, Serum PT PTT RBC Count Renal Function Pan RPR STAT Stool, Culture T3 Uptake T4 Thyroid Cascade Prof TSH Urinalysis Urinalysis, Complete Urine, Culture WBC Count WBC Diff Hep B Surface Ab C-Reactive Protein Uric Acid, Serum HB Solu + Rflx Frac Physician Read Pap PERFORMANCE WORK STATEMENT: (A) GENERAL REQUIREMENTS: 1.The vendor must be CLIA certified. 2.Vendor must supply lab test request forms, tubes, specimen cups, slides, and all other necessary laboratory supplies to the Utah National Guard for use on this contract. 3.Retrieval of lab results must be available through the internet. 4.Most of the testing will be done on weekends. 5.Vendor must accept the government purchase card (VISA) for payment. 6.The government reserves the right to add tests to this contract as needed. (B) SPECIFIC TASKS: 1.An estimated quantity for each test required will be provided by phone to the vendor approximately 3 days prior to the testing. The oral order will be followed by submission of the Lab Test Request Form indicating the the exact number of each test required. 2.Specimens are to be picked up from two locations: a. UTARNG MEDICAL DETACHMENT, 17800 S Camp Williams Road (Redwood Road), Riverton, UT (POC: SSG Troy Thompson, 801-878-5075 or SFC David Openshaw, 801-878-5070). b. ARMY AVIATION (Airport #2), 7602 South 4470 West, West Jordan, UT (POC: SFC Brian Wardle, 801-816-3435 or SSG Desiree LeJeune 801-816-3607). 3.Routine tests should be reported within 72 hours. Results are to be faxed to 801-878-5144, Attn: SSG Troy Thompson via the force printer. All abnormal lab values are to be called to SSG Troy Thompson at 801-878-5075. EVALUATION FACTORS: The contract type for this procurement will be fixed price and award will be made on the basis of best value to the government, price and other factors considered. Selection will be based on the following evaluation factors, in order of importance: (1) offerors capability to provide the services required; (2) past performance, caliber of offerors performance on previous contracts of a similar nature; and (3) total cost or price. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the options. INSTRUCTIONS: Offers shall include (1) a quote for the base year and each option year for each of the required tests; (2) list of at least three of the most current past performance references for similar work covered by this solicitation. Provide the following information for each reference: (a) name and telephone number of contracting officer; (b) date of contract; and (c) brief description of the contract; (3) completed provision at 52.212-3, Offeror Representations and Certifications Commercial Items, or a completed registration on the ORCA website at https://orca.bpn.gov. Failure to provide the above listed items with your offer, may cause your offer to be considered non-responsive. ADDITIONAL INFORMATION: In accordance with FAR 52.212-1(k), prospective awardee shall be registered and active in the CCR database prior to award.
