- The Sheridan VA Medical Center, located at 1898 Fort Road, Sheridan, Wyoming, is soliciting offers to lease property for a VA Outreach Clinic in Afton, Wyoming. Approximately 1500-1800 square feet of medical clinical space is desired, and property must be located within a 5-mile radius of the Star Valley Medical Center in Afton, Wyoming. Ideal space would include, but is not limited to, reception/waiting area, triage room, 4 exam rooms, interview room, break room, storage room, oxygen storage room, clean and dirty storage room, and patient and staff bathrooms. The VA is issuing this Request for Proposal in accordance with FAR Part 15 where technical capabilities and past performance are of greater and/or equal importance than cost and/or price. Selection criteria, in descending order of importance, include: (1) Location of Proposed Site; (2) Technical Factors; (3) Time & Date of Facility Availability; (4) Price; (5) Past Performance; (6) Promotion of Energy Efficiency and Use of Renewable Energy. Solicitation documents will be available electronically on or about Monday, January 12, 2009 at the FedBizOpps website at http://www.fedbizopps.gov. Solicitation documents can also be requested via email at jason.brown5@va.gov or by standard mail by contacting Jason Brown, Contracting Officer, at (307) 672-1680. A site visit is scheduled for Thursday, February 5, 2009, to the Afton facilities of prospective Lessors. Upon review of the solicitation documents, prospective offerors are encouraged to provide questions and comments, in writing, in regards to the solicitation documents prior to the scheduled site visit. Solicitation number VA-259-09-RP-0111 has been assigned and should be referenced on all correspondence regarding this announcement and solicitation. Interested firms are reminded that in accordance with FAR Part 4.11, prospective contractors are required to have an active registration in the Central Contractor Registration (CCR) and FAR Part 4.12, prospective contractors must complete electronic on line annual representations and certifications (ORCA) databases prior to award of a contract. Both can be accessed at the Business Partner Network (BPN) website at http://www.bpn.gov/.
