- This is an attempt to gain information for a Maintenance Contract. Request for Information and your company may provide a Quote if the requirements can be met in the Statement of Work below. Respond date: 1/22/09. Statement of Work An Annual Maintenance Agreement is requested to provide a Maintenance Plan to support the Medical Center's lab Slide Stainer. The plan will provide training for one Biomed Tech to allow troubleshooting and repair of the stainer. Instrument Type is the Benchmark LT, Staining Module SN 610264. The contract will cover one year and option years may be quoted. The following features will be included: 1. Factory level training for one VA Biomed Technician with annual re-certification provided by the Contractor to be completed within 90 days of the Agreement effective date. 2. Training to include travel costs (airfare, lodging and most meal expenses, plus any and all material required to successfully complete the training). 3. Any specialized test equipment required to troubleshoot or repair the stainer is to be provided on consignment to the VA. 4. All parts necessary to complete an annual PM are to be provided. VA Biomed Technician shall perform the annual PM. 5. On site back-up support including all parts necessary to complete repairs will be provided by the Contractor in the event the VA cannot affect the repairs. In the event VA does complete the repairs, Contractor will provide all parts at no cost. Shipping and handling charges for parts provided in this cost. 6. The Contractor will provide unlimited applications troubleshooting during normal business hours 8am - 4:30pm Monday through Friday. 7. The Contractor will provide five days of application support (i.e. instrument move assistance, protocol assistance and on-site operator training) during normal business hours. 8. This service will apply to the slide stainer and all components including the CPU, printer, monitor and label printer. VA Computer Security Protocol will be adhered to with all computer peripherals.
