- Synopsis:Sources Sought Notice only. This is not a request for quotes or proposals. No formal solicitation document exists at this time. Seeking Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB) able to provide Quality Control (QC)l tests of linear accelerators, diagnostic units and patients dose calculations for the Brooklyn VA Medical Center located at 800 Poly Place, Brooklyn, NY 11209 and the St. Albans Medical Facility located at 179th St. & Linden Blvd., St. Albans, NY 11425. The contractor shall be knowledgeable regarding the Joint Commission on Accreditation of Healthcare Organizations JACHO, NY State, NY City, FDA, ACR, AAPM standards for quality of the X-ray units and linear accelerators. Contractor must be board certified by the American Board of Radiology (ABR) or the American Board of Medical Physics (ABMP) for diagnostic and radiation therapy and have liability insurance. Contractor is responsible for providing all calibration, scanning and survey equipment necessary for the successful completion of the tasks detailed in the Statement of Work. Contractor must be available during the JACHO visit. Contractor shall furnish labor, equipment and material required to perform the necessary tasks. Contractor shall provide number of years experience in performing the annual QC of linear accelerators and QC of the X-ray units. Contractor shall provide certificate and educational degrees. The contractor must possess all licenses and permits required to provide the services in accordance with federal, state, and local regulations. The NAICS code is 811219 and the small business size standard is $7.0 million. Responses to this notice should include the following: Name and address of company/provider (phone number and email of point of contact); indicate whether or not they are a Service-Disabled Veteran-Owned Small Businesses (SDVOSB) or Veteran-Owned Small Businesses (VOSB) and include experience in QC for linear accelerators, X-ray units, etc. and any other essential information demonstrating contractor's ability to meet the above requirements. If this project is set-aside for SDVOSB or VOSB, 51 percent of services shall be provided by SDVOSB or VOSB.
