January 14, 2009 Atterbury and IndyPendence Job Corps Center 1025 A Hospital Road P O Box 187 Edinburgh Indiana 46124 RFP:Center Food Service Operations REQUEST FOR PROPOSALS To Whom It May Concern: Atterbury Job Corps Center would like to offer you the opportunity to provide the food needs for our satellite Center located at 222 E. Ohio Street, Indianapolis, Indiana. The dining facility must be located within 2 blocks of the Center. The estimated number of students to use the facility daily is 160. Complete RPF packages can be obtained by email (gushwa.patty@jobcorps.org) or fax *812-314-6290), no telephone request will be accepted. We will require the following for your proper proposal: 1.Bid on RFP – Center Food Service Operations 2.We will require dated signature sheets to be provided with invoices of all students who used the dining facility at each meal. 3.This period, March 2, 2009 through February 28, 2010, is subject to the funding appropriation of the US Department of Labor. 4.As necessary, we will conduct several rounds of negotiations. 5.The total solution for Atterbury Job Corps Center is of most importance with our vendor choice – this includes service, past history, pricing, commitment of company and representative, cost, and solutions. 6.Provide status of your company type and size and be able to verify/certify (Ex.: 8A, Small Women Owned Business, Small Disadvantaged Veteran Owned Business, HubZone, Small Minority Owned Business, Small Disabled Veterans Business, etc.). This will be a part of vendor selection. If your company works with other companies such as this, provide your own pricing, as well as another column for this pricing in the MS Excel pricing spreadsheet. If you list two different pricing strategies in our bid, make sure that you follow the exact layout in the spreadsheet. Provide one list with your pricing and the other list (your partnership with any certified business, as listed above). 7.Outline how your company addresses Green Procurement according to the Federal Acquisition Regulation (FAR) (11.002, 11.303, 13.2, 23.2, 23.4, 52.223-1, 52.223-2, 52.223-15, and 52.223-16). For more FAR details, go to www.arnet.gov.far. 8.Outline how your company addresses Buy American according to the FAR (FAR 25.1, 25.2, 25.501, 25.11, 25.504-1, 52.225-2, 52.225-3, 52.225-4, 52.225-9, 52.225-10, and 52.225-11). For more FAR details, go to www.arnet.gov/far. 9.Outline your recycling program for any services you offer. 10.Atterbury and IndyPendence Job Corps are operated by Adams and Associates, Inc. a Federal Contractor for the U.S. Department of Labor – Job Corps. 11.Vendors are expected to closely read the terms and conditions and provide binding signatures as applicable in each attachment. 12.The successful vendor shall procure and maintain the proper insurance from a carrier licensed to do business in the states specified in this RFP. A certificate of insurance evidencing said coverage shall be supplied by the vendor naming Adams and Associates, Inc. and specified Job Corps Center as an additional insured under the liability policy. The liability policy shall contain a provision that such policy shall not be cancelled until thirty (30) days prior written notice of cancellation has been received by Atterbury Job Corps Center. 13.Vendor shall provide a profile of the company, such as: Ownership; Location of offices; Number of employees; Company background and history; Company qualifications as a superior service provider for this RFP; and Resumes for key staff to be responsible for contract performance. 14.Pricing shall remain firm for the period agreed/quoted. 15.In case of default by the winning vendor, Adams and Associates, Inc. may procure the product (s) or service (s) from other sources and hold the vendor responsible for any excess cost occasioned thereby. 16.Vendor must provide full time ongoing customer service support for any questions, challenges, and needs. This must be available to the customer at no additional cost. This includes a toll free number. 17.Payment will be made on items purchased with net 30-day terms. 18.The vendor, if a contract is executed, affirms that he/she is familiar with the conditions under which the work is to be performed. 19.We will provide a subcontract for your signature and our signature that will encompass all of these specifications, terms and conditions, FAR clauses and other areas. This will be the only agreement signed by Adams and Associates, Inc. and the winning bidder. A.Service Questions •Who inside your organization would be the main contact for Adams and Associates, Inc. and each Job Corps Center regarding customer service issues, orders, and questions? B.Pricing, Contracts and Billing Questions •Who inside your organization would be the main contact for Adams and Associates, Inc. and Atterbury Job Corps Center regarding additional needs? •Detail your capabilities for reporting. Include sample reports. •Please describe your process for issuing credits for billing disputes. •Detail any additional services, as it relates to the RFP, which would result in any additional charges if any. C.General Conditions •The Subcontractor shall at all times abide by the Center's rules and regulations and security regulations in effect and shall be observed. •No weapons, alcoholic beverages or controlled substances are to be consumed on or transported to the site. •There shall be no fraternization with the students or staff at the site. •Specified work hours shall be strictly observed.
