- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitations and a written solicitation will not be issued. This solicitation is issued as a Request for Proposal (RFP). This procurement is for the purchase of the following: 1 Each: SonoSite TITAN High Resolution Ultrasound System (CPD, DCPD, AND Velocity Color) Model No.: L04246. 1 Each: 4-2 mHz, Microcurved Array, Model No. p04102-C15e. 1 Each: Mobile Docking System (MDS) Lite, Model No. P08050. 1 Each: TITAN SonoSite Ultrasound System User Guide, English, Model No. P03322. 2 Each: TITAN SonoSite Ultrasound System Service Manual, English, Model No. P03309. 1 Each: Standard - 12 Months - Standard Protection Warranty (TITAN). Please include shipping with the quote. The destination for this item is CSTARS Missouri, 1320 South Grand Blvd, St. Louis, MO 63104. The delivery date of these items is 30 days ARO. This is an intention to Sole Source to limited sources. The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-29(effective 19 January 2009) and Defense Federal Acquisition Regulation Supplement change notice 20090115. FAR 52.212-4 and 52.212-5 applies to this acquisition. WAWF procedures also apply. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. FAR 52.212-2, Evaluation - Commercial Items, applies. Technical capability will be evaluated by how well the proposed product meets the government requirement instead of predetermined sub-factors. Evaluation Basis for Award: Award will be made to the offeror who has the lowest priced, technically acceptable proposal. It is the offeror's responsibility to be familiar with applicable clauses and provisions. The NAICS Code is 334510 the size standard is 500 people. The type contract anticipated is firm-fixed price. A due date of offers will be 12:00 PM, Central Time, 20 Feb 09.
