Description
The Grafton Job Corps Center would like to sub-contract for Wellness Services to be provided through our wellness center on our North Grafton Campus. The scope of services required is listed below. SCOPE OF SERVICES The wellness services to be provided under this subcontract shall conform, at a minimum to health and weUness services specified in the Job Corps Federal Regulations (Title 20 Code of Federal Regulations, Part 638.510), in the Job Corps Policy and Requirements Handbook (PRH), Chapter 5 and applicable Technical Assistance Guides (TAGs). To accomplish these ends, the Center Physician shall: A. Provide wellness services for a maximum of twelve (12) hours per week. Any additional hours must be approved in advance by the Center Director. The services will be provided at the facilities located at the Center Wellness Services Clinic. Center staff will provide clinical services in emergency situations only, except as provided in written health care guidelines. The Wellness Center is not authorized to provide routine treatment to staff The maximum weeks of service will not exceed 48 weeks per year. Services are not required during DOL-mandated Center shutdowns that occur during the summer (2 weeks in July) and winter (2 weeks in December). The Subcontractor will not be paid during this period, unless the Physician is required by the Center to provide services, including call, or attend Center-mandated training during this shutdown. The Center will notify the Physician of planned shutdowns as early as practicable. B. Develop, implement and monitor the total Center Wellness Program. Specific duties include, but are not limited to: 1. Advising the Center Director and Wellness Services Staff on all medical and health related matters, including, but not limited to: staffing requirements; student medical terminations, including the furnishing of diagnosis, prognosis, and recommendations for future care; utilization and cost of services; personalization of patient care; and any other health areas deemed appropriate by the Center Physician and/or the Center Director. 2. Consulting with Center staff on ordering, accounting for and safeguarding health equipment, supplies and medications. 3. With due regard for Job Corps requirements, prioritizing student health needs and setting program direction accordingly. To accomplish this end, the Center Physician shall confer with the Center Wellness Services Manager (or chief nurse - -¬ or medic), the Center Dentist and the Center Mental Health Consultant as required. 4. Managing health care costs. To accomplish this end, the Center Physician shall meet with the Center Director or Center Finance & Administration Director on a quarterly basis to review expenditures for the prior periods and establish restraints, if necessary, to attempt to stay within budgetary guidelines. However, the best interest of student health will supercede budgetary guidelines. The Center Physician may involve the Job Corps Regional Health Consultants in this process, if needed. 5. Developing a system to promote the coordination and integration of the health and wellness components (Medical, Dental, Mental Health) with other Center activities. This will be done in cooperation with the Wellness Services Manager, the Center Dentist and the Mental Health Consultant. 6. Advising staff on a system that would ensure the maintenance, confidentiality and safeguarding of all student health records in the Wellness center. 7. Issuing and maintaining signed and dated health care guidelines for medical care in accordance with PRH Chamber 5 and TAG 6-Q. C. Provide direct medical services to students, including: 1. A cursory medical inspection for each student within the first 48 hours after arrival on-Center will be performed by the Center R.N. or L.P.N. Any student whose health is felt to be compromised will be referred to the physician immediately. A definitive medical examination and review of health history of each student will be performed within 14 days of arrival on-Center. The Wellness Services Manager will be responsible for scheduling examinations within 14 days of arrival. Off-site scheduling may be necessary to accommodate the number of entering students. The medical examination will include the interpretation of all routine laboratory tests and, when necessary, X-rays. (Fees for consultation by a radiologist are not included in this agreement.) 2. For diagnosis and treatment of illness and injury, the Center Physician will be available on-Center for sick calls of appointments as scheduled by the Center Director or his/her representative. 3. Arrangements for preventive immunizations and detection and control of communicable diseases as specified in PRH Chapter 5.1, section Rl.d: Immunizations. 4. Direction of Center staff in obtaining all necessary off-Center medical referrals for specialist and institutional care with Center Director ofWellness Services Manager approval. If the Center Director or his/her agent fails to obtain medical care for a student in a timely fashion after consulting with the physician, the Center will assume all resulting liability. 5. Ensuring that medical reports and vouchers are accurate, reasonable in amount and delivered to the Center in a timely manner.
