Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is IHS-SOL-0900059 for Medical Waste services. This solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-29, Amendment 1. The NAIC code is 562211 and the small business size standard is $12.5M. Medical Waste Pickup and Disposal services Period of Performance February 1, 2009 or date of award through January 31, 2010 Evaluation will be based on price and past performance Price Schedule for Waste Disposal Services Waste Disposal Service for 12 months at $________ per month = $______________. Past Performance is ability to provide documentation of past performance of satisfactory service performed in a federal facility or hospital. The offeror shall provide past performance references with name, contact name, period of performance and telephone number. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations and Certifications – Commercial Items with its offer. The clause found at FAR 52.212-4, Contract Terms and Conditions – Commercial Items applies to this acquisition. The clause found at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders – Commercial Items applies to this acquisition. STATEMENT OF WORK: Contractor will pickup and dispose of medical waste for the Claremore Indian Hospital, 101 S. Moore, Claremore, and OK 74017 two times per week. Contractor shall provide reusable Rubbermaid leak proof containers with liners, approximate size of 12.5” x 22.5” H. Lid dimensions 24 x 6”. The Contractor shall provide required tracking manifest method of labeling of each container for transport and incineration of hospital waste with copies to the Claremore Indian Hospital Contracting Officer’s Technical Representative, Charles Jones and 918-342-6482. The Contractor shall provide and show proof of insurance coverage to protect the U.S. Government against all claims arising from the handling of the waste from the Claremore Indian Hospital to the point of incineration. The Contractor shall provide to the Claremore Indian Hospital a letter indicating the employees working for the Contractor has had adequate training in handling “medical waste”. The Contractor shall provide proper handling of waste, until incineration in a manner not to reflect back to the Claremore Indian Hospital. The Contractor shall only use specifically approved vehicles for the transport of medical waste from the Claremore Indian Hospital. The Contractor will provide training for the health care workers in determining what are “medical waste” and the handling of such waste. Initially 14 containers per week will be needed, with additional containers available when telephone contact is made by the Contracting Officer’s Technical Representative (COTR) or designee. Claremore Indian Hospital is a tobacco free facility.
