Description
The Oklahoma City VA Medical Center, 921 N.E. 13th Street, Oklahoma City, OK is issuing a solicitation for Specialized Medical Resource Sharing Agreement for Primary Care Services Community Based Outpatient Clinic (CBOC). Services shall be for an estimated 650 veteran patients residing within a 30 mile radius of Enid, Oklahoma. It is the intent of the Oklahoma City Department of Veterans Affairs to utilize a pre-existing clinic that is JCAHO accredited or operating under the mandates of JCAHO within 30 miles of this geographical area. The following services are included in the solicitation: Primary, post hospital care, behavioral medicine, radiology, laboratory services and urgent first fill prescriptions to treat acute conditions. Services of the CBOC shall be made available five days a week Monday through Friday during normal clinic operating hours. Responsibilities and operating procedures of the contractor are in the Provider Handbook, part of which are provided in the solicitation package within the statement of work. The North American Industrial Classification System (NAIS) is 621498 and the size standard is 5M. It is the VA's intension to negotiate and award a single contract for Primary Care Services. This contract will be awarded on a competitive basis under the authority of Title 38 U.S.C Section 8153 pending availability of funds. The contract will be for a period of one year from the date of award with four one year renewal options that can be exercised at the option of the Government. Costs for primary care services should represent an "all inclusive" reimbursement for routine delivery of primary health care. Offerors should quote a capitation rate per enrolled patient for a period of one month. The yearly costs should include the cost per patient per month x 12 months x estimated number of patients. Bids must be submitted for the entire contract period and must be complete in order to be considered. Requests for bid packages must be submitted in writing only to Beverly.Kimball@va.gov or via fax to 405-456-5123 to the attention of Beverly Kimball. All requests for bid packages must be on company letterhead. Responses to questions shall not be provided after Monday, February 2nd. Bids shall be due back in this office no later than 4:30pm on Friday, February 20, 2009.
