Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. DESCRIPTION: Cost per copy maintenance for Government owned copiers. This requirement is for one (1) year from date of award with four (4) option years. PERFORMANCE WORK STATEMENT: This is a Cost Per Copy maintenance contract for Government owned copiers to include: a.On-site service/support 24 hours a day, 7 days a week, with a 2 to 4 hour response time on all calls to locations throughout the state of Utah. b.Cost per page all costs to be calculated by page to eliminate unknown or changing costs. The cost per page must include support for everything but paper. Preventative maintenance should be performed on all machines whenever they are seen. 2. A quote sheet is provided which includes a list of copiers (approximately 100), the estimated number of copies per year and location. Contact Sherri Shepherd at sherri.shepherd@us.army.mil for quote sheet. a.The estimated number of copies was based on approximate usage for last year. This is only an estimate and the Utah National Guard will only pay for copies actually used. There is no minimum number of copies guaranteed. 3. Contractor must be certified to perform service on all machines included on the contract. 4. The Government requires the capability to add or delete machines based on the availability of funds. Machines will only be added or deleted by modification to the contract. Machines will be added at the contracted rate, and the rate for any new models, which are not on the contract, will be negotiated. 5. The billing cycle for this contract will be the last working day of each month. Vendor will submit a single invoice itemizing all machines to include model, serial number, and number of copies used for each copier. 6. The contract will be for one year beginning o/a 1 March 2009, with an option to renew for four additional years. APPLICABLE PROVISIONS AND CLAUSES: The following clauses and provisions are incorporated by reference. (15) DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. The following clauses within 252.212-7001 apply to this solicitation and any resultant contract: (a) DFARS 252.232-7003 Electronic Submission of Payment Requests; (b) DFARS 252.247-7023 Alt III Transportation of Supplies by Sea. (16) DFARS 252.232-7010 Levies on Contract Payments. EVALUATION FACTORS: The contract type for this procurement will be fixed price and award will be made on the basis of best value to the government, price and other factors considered. Selection will be based on the following evaluation factors, in order of importance: (1) offerors capability to provide the services required; (2) past performance, caliber of offerors performance on previous contracts of a similar nature; and (3) total cost or price. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the options. INSTRUCTIONS: Offers shall include (1) a quote for the base year and each option year on the quote sheet provided; (2) documentation to show technicians are certified to perform work on all machines covered by the contract; (3) list of at least three of the most current past performance references for similar work covered by this solicitation. Provide the following information for each reference: (a) name and telephone number of contracting officer; (b) date of contract; and (c) brief description of the contract; (4) completed provision at 52.212-3, Offeror Representations and Certifications Commercial Items, or a completed registration on the ORCA website at https://orca.bpn.gov. Failure to provide the above listed items with your offer, may cause your offer to be considered non-responsive. ADDITIONAL INFORMATION: In accordance with FAR 52.212-1(k), prospective awardee shall be registered and active in the CCR database prior to award. Offerors may obtain information on registration at www.ccr.gov. Offers shall be received NLT 10:00 A.M. on 9 February 2009.
