Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation, FA2517-09-T-5016, is being issued as a Request for Quotation (RFQ). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. (iv) This procurement is a small business set aside. The NAICS code is 238990 All Other Specialty Trade Contractors and the size standard is $14 million. (v) Contractor shall utilize the below line item structure when preparing quotes: Line Item 0001 Description: Appliance Preventative Maintenance-Dorms Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform preventive maintenance on 95 washers and 110 dyrers in 4 dorms In Accordance With (IAW) attached Performance Work Statement (PWS). Line Item 0002 Description: Appliance Service, Repair, and Parts-Dorms Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform service and repair, and to provide parts on 95 washers and 110 dyrers in 4 dorms IAW attached PWS. Line Item 0003 Description: Appliance Preventative Maintenance-Lodging Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform preventive maintenance on 55 Stackable Washer/Dryers, 64 Stove/Ovens, 64 Dishwashers, 4 Ice machines, and 231 Refrigerators IAW attached PWS. Line Item 0004 Description: Appliance Service, Repair, and Parts-Lodging Non-Personal Services to provide all labor, parts, tools, materials, equipment, and personnel necessary to perform service and repair, and to provide parts on 55 Stackable Washer/Dryers, 64 Stove/Ovens, 64 Dishwashers, 4 Ice machines, and 231 Refrigerators IAW attached PWS. (vi) The following commercial item is requested in this solicitation: Non personal services to provide all labor, parts, tools, materials, equipment, personnel, supervision, transportation, and other items or services necessary to perform maintenance and repair on appliances in the Dorms and Lodging at Peterson AFB, CO IAW the attached PWS from 1Feb 2009 - 31 Jan 2009. (vii) Advanced payments are not authorized. To be performed on Peterson AFB, CO from 1 Feb 09 - 31 Jan 09. (viii) The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors/Commercial Items. All firms must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. All firms must be registered in the Central Contractor Registration Database at www.ccr.gov in order to be considered for award. (ix) FAR 52.212-2(a) Evaluation - Commercial Items, applies to this acquisition. Quote will be evaluated based on price. (x) The provision at FAR 52.212-3, Offeror Representations and Certification-Commercial Items, is applicable to this acquisition. All vendors must be registered in Online Representations and certifications (ORCA) (https://orca.bpn.gov) at the time of Quote submittal. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to the acquisition. The quote should provide for a cost of each line as described in paragraph (v). (xv) Site visit will be conducted at 0900 on 20 January 2009.
