- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA7000-09-T-0029, in accordance with FAR Parts 12 & 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-29, 14 Nov 08. The North American Industrial Classification System (NAICS) number is 339112 and the business size standard is 500 employees. The item required (CLIN 0001) is a Biodex System 4 Pro Patient Positioning System with motorized seat height, fully assisted Dynamometer height adjustment, front-to-back and side-to-side chair adjustment. The item must include at a minimum a one year warranty to include parts and labor. At least two operators manuals and one service manual must be included. The vendor is also responsible for providing on-site training and setup at the customers convenience. The USAF Academy has a Brand Name Only requirement. The 10th Medical Group has determined this to be the only product that meets all its required features and functionality based on the following justification. Biodex is the only company that offers the isokinetic tester. The unit requested operates at three different speeds to test shoulders, wrists, knees, ankles and the back. It gives an accurate assessment of functional capability. This equipment is valuable to USAFA due to its capacity to accurately test strength discrepancy in post-operative extremities. In addition the Biodex will allow USAFA providers the ability to screen cadet candidates who have pre-existing surgical procedures and provide an accurate view of functional ability prior to acceptance into USAFA. This proposed firm fixed price acquisition is a 100% Total Small Business set-aside. All responsible sources may submit a response, which if received timely, will be considered by the 10th Contracting Division. The Government does not intend to pay for information provided under this synopsis. Delivery is to be made to the 10th Medical Group at the U.S. Air Force Academy, CO 80840, FOB Point: Destination. Delivery is required 5 weeks after receipt of order. This product must comply with the Buy American Act (BAA) (100% manufactured in the USA). The following Federal Acquisition (FAR) clauses apply to this combined synopsis/solicitation and offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors--Commercial Items, addenda applies: The contractor shall submit their quote on company letterhead to include the following: solicitation number, the time specified in the solicitation for receipt of offers, contact name, address, telephone number of the offeror, unit price (shipping cost included in unit price), total price, any discount terms, cage code, DUNS number, tax identification number, size of business, warranty information and a statement that your company is in compliance with the provisions at FAR 52.212-3 (ORCA) (http://orca.bpn.gov/publicsearch.aspx) and DFARS 252.225-7000 (http://farsite.hill.af.mil/), Buy American Act-Balance of Payments Program Certificate, which must be signed and returned with the quotation. In addition, any acknowledgement of solicitation amendments must be included. In accordance with FAR 52.212-4, contractors must be registered with the Central Contractor Registration to conduct business with the Department of Defense. No purchase order can be awarded to any company without this registration which can be completed at www.ccr.gov. Quotes must be received NLT 1600 Mountain Time, 17 February 2009 at 10 LGCB, Attn: Matt McLean, 8110 Industrial Drive, Suite 200, USAF Academy, CO 80840-2315. Email or fax quotations are acceptable and can be submitted to the below mentioned individuals. Offers received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. The Government will award a contract resulting from this solicitation to the responsive responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: award will be made on the basis of determining lowest evaluated price. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be acceptable, and therefore will not be considered. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
