Description
Contractor shall provide qualified staffing to perform and supervise Hemodialysis and Peritoneal Dialysis services to beneficiaries of the VA Healthcare Network Upstate New York (VAHNUNY) at Bath. Bath VA has Outpatient clinics located in Elmira and Wellsville. Services to be provided under this contract shall be provided at the Contractors site(s) at various geographical locations throughout the VISN 2 service area, utilizing Contractor owned and maintained equipment. Contractor's facility(s) must be located within the VISN 2 service area. It is the intention of the VA to award contracts to Offerors who have facilities located near the Bath VA Medical Center and/or any of the outpatient clinics listed above. Contractor shall provide outpatient, Hemodialysis and Peritoneal Dialysis services to beneficiaries of VISN 2, at the Contractors site(s), utilizing contractors staff and equipment and patient-owned equipment for Peritoneal Dialysis. The health care provided will be delivered in a manner consistent with the needs of the patients and standards of quality delivered to the contractors non-VA patients. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. PRICE: The per treatment price in the Fee Schedule is an all inclusive rate which includes all equipment, facility, nursing, technical personnel, supplies, lab tests, professional fees and drugs that are necessary to furnish dialysis treatment in an outpatient setting. Outpatient treatments for VA patients must be authorized by the VA before treatment begins. The need for any additional service shall be documented in the patients medical record and included in the ICD9-CM diagnosis code.
