Description
The Fargo VA Medical Center is hereby seeking sources for primary care at a Community Based Outpatient Clinic located in Williston North Dakota to provide services for VA beneficiaries. There are currently 830 veterans enrolled for this site. The current market penetration is approximately 45%, and the VA anticipates an approximate 2% increase over the next several years as new users enroll for care. It is also estimated that each patient will have an average of 3 visits per year. The anticipated period of performance is November 1, 2009 through October 31, 2010 with four, one-year option periods to be renewed at the Government's discretion. Contractor shall provide Joint Commission (JC) Certified medical facilities, health care providers, medical equipment, supplies and administrative support. The VA prefers, and highly encourages that the contractor's providers are exclusively assigned for VA patient care, and must provide services for a minimum, Mon-Fri (8:00am - 4:30pm) excluding federal holidays. The number of providers / support staff will be based on the VA's model in relation to the patient workload. Compensation shall be made through monthly capitation payments for each patient in accordance with the terms and conditions of the resulting contract. It is imperative that interested parties respond to this sources sought, as the responses to this announcement will be used to determine if an interest exists, and thereon, the likelihood of a successful solicitation. Respondents must indicate that they currently, or are willing to meet the aforementioned requirements. If adequate responses are received, a solicitation (request for proposals) will follow in the upcoming months. No telephone requests will be honored. Questions must be submitted in writing to the contracting officer. If contractors will submit questions, they are strongly encouraged to do so within ten (10) calendar days after issuance of this sources sought announcement. Verbal questions will not be addressed. Potential parties must respond indicating their interest no later than March 12, 2009. NAICS Code for this requirement is 621498. Size standard is $10.0 mil.
