Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for an OB-GYN to provide services at the White Earth PHS, Health Center in Ogema, Minnesota. The period of performance will be from Date of Award through September 30, 2009 or 176 hours; whichever comes first. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-09-056, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-09-056 and provide a postal mailing address, email address and state preference as to where the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be March 13, 2009. The closing date of the RFQ package is March 30, 2009. This is small business set-aside.
