Description
USDA Forest Service, Cherokee National Forest has a requirement to furnish, transport, and stockpile crushed aggregate to Wolf Creek Parking Lot, Cocke County, Limestone Cove Parking Lot, Unicoi County and Rock Creek Recreation Area, Unicoi County, on the Nolichucky Ranger District. PROJECT TITLE: Furnish, Transport and Stockpile Aggregate at 3 locations on Nolichucky Ranger District. This solicitation, identified as AG-4756-S-09-0014 is issued as a Request for Quotation using simplified acquisition procedures. Issue date of this synopsis/solicitation is March 3, 2009. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are requested and a written solicitation will not be issued. SET-ASIDE: This solicitation is a Total Small Business Set-Aside. The applicable NAICS code is 484220; the applicable small business size standard is $23.5 million dollars average annual receipts for the preceding three fiscal years. DESCRIPTION OF REQUIREMENT: Contractor shall furnish, deliver, and stockpile crushed aggregate to Wolf Creek Parking Lot, Cocke County, Limestone Cove Parking Lot, Unicoi County and Rock Creek Recreation Area, Unicoi County. Aggregate provided shall be one or any of the following TDOT gradations: crusher run (TDOT grade D), surge stone or #10 chat. Quantities and gradations to be delivered to the following locations are as follows: A. 1.)Wolf Creek Parking -- Surge - 60 Ton$______/ton $_________total price A. 2.)Wolf Creek Parking -- Crusher Run - 240 Ton $_____/ton$_________total price B. Limestone Cove Parking -- Crusher Run - 640 Ton $_____/ton$_________total price C. Rock Creek Parking --#10 Chat -785 Ton$_____/ton$_________total price GRAND TOTAL ITEM A-C $_______________ Method of Measurement shall be AQ (Actual Quantity) Unit price will include transportation costs and any applicable sales tax. The materials and their use shall comply with TNDOT specifications for highway use. The aggregate sources shall be approved by the Forest Service before any materials are delivered to the site. Weight tickets from sources listed shall constitute a certificate of compliance for quality requirements. The weight of the aggregate shall be reduced by moisture content in excess 7%. LOCATION: Directions to the three locations are as follows: Directions to Limestone Cove From Unicoi, take TN 107 east 3.5 miles to Limestone Cove. The parking area is on the right. Directions to Rock Creek From I-26 take exit 34 to Main Street (TN107) in Erwin, take a left on TN 395 east for 3.5 miles to Rock Creek. Directions to Wolf Creek Parking From the intersection of TN 107 and US 25/70, travel northeast on US 25/70 for 3.7 miles. Turn right on Wolf Creek Road, travel.2 miles bear right on FDR 96D. Travel.6 miles the parking area will be on the left. DELIVERY: Delivery shall be determined by the COR at the time of issuance of the Notice to Proceed and will be dependent on weather. Placement shall be as directed by Forest Service on-site representative. Delivery shall be made Monday through Friday between 8:00 a.m. and 5:00 p.m. unless prior arrangements have been agreed upon. The period of this contract shall be from date of issuance of Notice to Proceed thru Sept. 30, 2009. Desired date for work to begin is March 30, 2009. EQUIPMENT: Trucks used for hauling shall have a clean, tight bed and a minimum payload capacity of 20 tons. INSPECTION: Inspection of facilities and equipment shall be allowed during all segments of the total process. Acceptance of the in-place aggregate shall be contingent upon approval by the Forest Service. Materials and in-place product shall comply with TDOT specifications. Certified weight tickets for the aggregate delivered and placed as directed will be the basis for payment. SUBMIT OFFERS: On company letterhead or form, no later than close of business March 19, 2009, by regular or express mail to: Contracting Office, Cherokee National Forest, 2800 North Ocoee Street, Cleveland TN 37312. Proposals may be emailed to: jadler@fs.fed.us. Proposals will be evaluated on price and past performance. A fixed unit price contract will be awarded from this solicitation. SPECIAL NOTE: Successful contractor must be registered in the Central Contractor Registration Database (www.ccr;gov) and in Online Representations and Certifications (http://orca.bpn.gov) prior to award of contract. Vendors must submit a DUNS number and taxpayer identification number with their quotation. Registrations in CCR and ORCA are not required for submission of proposal.
