- Attachment 6 - Proposal Preparation Instructions to Offerors Attachment 5 - Evaluation Criteria and Basis for Award Attachment 4 - CBSP DD Form 254 Attachment 3 - CBSP Past Performance Questionnaire Attachment 2 - CBSP Price Schedule Attachment 1 - CBSP Statement of Objectives RFP In accordance with FAR 12.603, Streamlined Solicitation for Commercial Items, the following information is provided. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is HC1013-09-R-0001. The solicitation is issued as a request for proposals (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-29 effective January 14, 2009. (iv) This is an unrestricted solicitation; it is not a small business set-aside. The Small Business Competitiveness Demonstration Program is not applicable. The associated NAICS code is 517410 (Satellite Telecommunications). The small business size standard for NAICS code 517410 is $15 million. (v) The list of contract line item numbers and items, quantities and units of measure for the base year and option years is included at Attachment 2, Schedule. (vi) The description of requirements is at Attachment 1, Statement of Objectives. (vii) Date of performance: The contract resulting from this RFP will be firm-fixed price and consist of one 12-month base ordering period with four additional 12-month option periods. Total length of the contract, if all options are exercised, is five years. The first ordering period is considered to be from the effective date of contract award through the end of the twelfth month after the effective date of contract award. Place of delivery and acceptance: Global. FOB point: Destination. (viii) FAR 52.212-1, Instructions to Offerors-Commercial Items (JUN 2008), is incorporated by reference and applies to this acquisition. Addenda are included. (ix) 52.212-2, Evaluation -- Commercial Items (JAN 1999), is incorporated substantially by full text. (x) Offeror shall include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items, Alternate 1 (APR 2002), incorporated by full text. NOTE: If your firm's representations and certifications have been entered into the http://orca.bpn.gov/, your firm is not required to submit additional representations and certifications. However, your firm must provide its DUNS number and a statement that the Online Representations and Certifications (ORCA) website has current information. The provision may be obtained at http://farsite.hill.af.mil. (xi) FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (FEB 2007), is incorporated by reference and applies to this acquisition. (xii) FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items (JAN 2009) (DEVIATION), is incorporated by full text and applies to this acquisition. Additional FAR clauses cited in the clause are applicable to this acquisition and are included in the RFP. (xiii) Contract award cannot be made to any contractor not enrolled in the Department of Defense (DOD) Central Contractor Registration (CCR) program. The CCR website is http://www.ccr.gov. Partial quotes will not be accepted. The Government intends to make a source selection for a firm-fixed price, single-award Indefinite-Delivery Indefinite-Quantity contract but reserves the right to make no award, depending on the quality of the proposals received and the availability of funds. (xiv) The resulting contract will not be a rated order under the Defense Priorities and Allocations System (DPAS). (xv) Questions are due no later than 9 March 2009, 10:00 AM Central Standard Time. RFP responses are due no later than 2 April 2009 at 10:00 AM Central Standard Time. Refer to Attachment 6 for details.
