Description
This requirement is for the United States Department of Agriculture (USDA) Animal Plant Health Inspection Service (APHIS). Ames, IA. APHIS/VS/NVSL requires Occupational Health blood and urine tests. Samples will be obtained for the most part on site, into sterile urine collection cups and transported back to the contracting lab in coolers. Some blood and urine samples may be obtained on site at the contracting lab. A listing of the required urinalysis, CBC, chemistry panels and serology is provided. Requirements: Collect blood and urine samples for approximately 800 employees on site. Most samples will be obtained over a concurrent 6 month period. An off site location for sample collection must be provided within a 10 mile radius of the NCAH, and must provide service 7 days a week 24 hours a day. The contracting laboratory must be CLIA certified and must adhere to all HIPPA standards and requirements. Contracting lab personnel will complete CLIA (Clinical Lab Improvement Act) Regulations competency based training and update the training in accordance with the CLIA standards. Contracting lab must be able to process samples within 2 hours of collection. Results of lab work must be made available to the Occupational Health Nurse within 3 business days for any routine labs, and within 2 weeks for labs that need to be sent out for further testing. For lab work needing to be sent out by the contracting lab for further testing, strict CLIA regulations must be followed, and the HIPPA standard applies. Any order issued during the effective period of this contract and not completed within that period shall be completed by the contractor within the time specified in the order. The contract shall govern the Contractor’s and Government’s right and obligations with respect to that order to the same extent as if the order were completed during the contract’s effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after date of last option period exercised. APHIS will award a firm fixed price definite quantity, indefinite delivery commercial supplies and services contract for one (1) base year and four (4) option years.
