- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81NTE90268500 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is set aside for small business under NAICS code 339113, Size STD 500. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: CLIN 0001, 5 EA VIGILANCE II CCO/Sv02 MONITOR; CLIN 0002 5 EA Should include 1 Year additional warranty; CLIN 0003, 10 EA OM2 Optics module; CLIN 0004 10 EA Cardiac Output Cables; CLIN 0005 5 EA VIG2S VIGILANCE MONITOR STAND; CLIN 0006 5 EA VIG2AP VIGILANCE ADAPTER PLATE. Vendors offering other than the brand name product shall meet or exceed the following Salient Charteristics: Hemodynamic Monitoring System with Continuous output (CCO), continuous end distolic volume (CEDV), and venous oxygen saturation (SvO2 and ScvO2 when used with appropriate Continuous Cardiac Output, Volumetric, and Oximetry Catheters. The display needs to provide and/or have: Cardiac or Oxygenation Profile Selection, Customized Trend, Data Relationship, and STAT Displays, Trend Scrolling for Historical Data Review, Bedside Monitor Interface for Parameter Display Convenience, Thermistor and Thermal Filament Test Ports. Should weigh under 8lbs with Dimensions roughly equal to Height 9.50in, Width 11.50 in, Depth 8.17in (exclusinve of navigation knob), with a TFT Display Active Area 8.3 in x 6.2in (211mm x 157mm) Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other FAR clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order. Place of delivery and acceptance is Brooke Army Medical Center, Bldg 3600, 3851 Roger Brooke Dr, Fort Sam Houston, TX 78234. Shipping FOB destination is preferred. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Vendors shall comply with FAR 52.211-6, Brand Name or Equal, if submitting other than the Brand Name product. Offers will be evaluated according to Far 52.212-2. Evaluation factors at 52.212-2 (a) are price and conformance to salient characteristics outlined in this solicitation.
