- Revised Physician Invitation Memo Revised Solicitation Package One 09-242-SOL-00003 Revised Solicitation Package Two 09-242-SOL-00003 The Albuquerque Area Indian Health Service (AAIHS) Santa Fe Indian Hospital, (SFIH) has a requirement for Physician services. The contract award will be for a base year period of 12 months, with four (4), one (1) year-optional renewals. It is anticipated that a Indefinite Delivery, Indefinite Quantity contract will be awarded with individual Task Orders issued against the base contract as needed. The prospective contractor shall provide after hours urgent care services at the Santa Fe Indian Hospital (SFIH) from 8:00 p.m. to 8:00 a.m., a total of 12 shifts per month. Offeor will manage a moderate volume walk-in clinic and urgent care department open 24 hours per day. The physician must manage a full range of common medical and pediatric problems, as well as to evaluate and treat a wide range of medical conditions which can result in admission to the Santa Fe Indian Hospital (SFIH) general medical-surgical ward. The physician shall also manage a wide range of inpatients from admission to discharge. Contractor will be asked to change his/her work hours depending upon management's needs and may exceed the normal hours. Contractor will not exceed an estimate 180 hours per month, this is to include weekends and federal holidays. Flexibility of the hours and the scheduled to be coordinated between the Government Project Officer and the Contractor. Perform duties during day, evening, night shifts as required by program needs and serve as "on call" as needed. Credentialing Requirements Physician contractors must apply and be approved for clinical privileges prior to providing patient care. Physician contractors shall submit the following to the service unit medical staff office: medical staff application with request for clinical privileges, copies of active unrestricted professional licensure, professional degree, residency and board certification in specialty, current curriculum vitae, proof of professional liability coverage, and DEA certificate. Projected Request for Proposal (RFP) release date is on/or about February 25, 2009. Please download the four file packets to Solicitation No. 09-242-SOL-00003 when requesting the solicitation package. Requirements must be addressed in writing only and may be e-mailed to Bradley.Platero@ihs.gov or faxed to attention of Bradley Platero at (505)248-4641 before the deadline time and date of 4:30 p.m on 04/07/09. Amendment No. 7 is hereby issued to replace Amendment Nos. One through Six and the initial #09-242-SOL-00003, which contained errors/corrections. Amendment No. 7 contains the entire revised document #09-242-SOL-00003. The new Request for Proposal due date is hereby extended to now read as: April 7, 2009.
