Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F6T3SV9070AZ01 is issued as a Request for Quote (RFQ). This request for quote is due by 19 Mar 2009, 9:00am Pacific Time. The RFQ will result in a firm fixed price contract. This is a 100% set aside small business. The North American Industry Classification System (NAICS) code for this acquisition is 722310. Size standard is $20.5M EVALUATION CRITERIA IS BEST VALUE: PRICE, MANAGEMENT APPROACH AND TECHNICAL ACCEPTABILITY. Offers will be evaluated on the afore mentioned criteria. Criteria includes meeting Statement of work and demonstrating a feasible approach to starting full operations by April 4th 2009. Period of performance is 1 April 2009 TO 31 OCTOBER 2009 PDF drawings and building layouts available upon request. A site visit is scheduled for 9:00 AM 17 Mar 2009. This acquisition is under Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items and Part 13.5, Test Program for Certain Commercial Items.. This requirement is for: SERVICES NON-PERSONAL: Furnish all labor, tools, equipment, material, transportation and supervision to: STATEMENT OF WORK: I.Description of Services 1.Program Overview A.The main concerns of this Statement of Work are meeting acceptable quality levels in the following areas: 1.Clean dining facility, which includes the dining room, clipper room and dishwashing room. Food and beverages spilled during the serving process or anywhere in the dining facility shall be cleaned immediately after the time of spill. 2.Clean equipment and utensils. This includes all food service equipment, serving line, and pans and utensils used in food service preparation. 3.Assist in preparation, replenishing and serving of food. B.Additional duties as detailed in the contract. 2.Contractor Personnel A.Management 1.The contractor shall furnish supervisory and direct labor personnel to accomplish all work required. 2.All employees of the Contractor employed in the performance of work under this contract shall be employees of the Contractor at all times and are not considered employees of the government. 3.An attendant supervisor, or other supervisor designated in writing to act in the contractor's behalf, shall be present during all hours when contract employees are working on the site. 4.All contractor employees shall be free from communicable diseases. The Contractor shall adhere to the Food Code Manual, US Public Health Service, FDA 2001, and all local codes and regulations of the State of Washington relating to food service sanitation and procedures. Medical Physicals, before starting work, and inoculations to include Hepatitis A vaccine. The contractor shall also insure that all dining hall attendants have a current State of Washington Food Handlers Card. The Food Handlers Card shall be renewed every three years. Physicals shall be renewed each year prior to the start of the new contract (or option years) Physicals and inoculations shall be at the expense of the contractor/vendor. 5.The contractor shall provide training necessary to employees and maintain records of all training on-site and have them available for Government review. In addition, the contractor shall ensure that the contract supervisor receives the food handler's training class from the Medical Squadron. This training will be conducted January drill on Sunday afternoon, once a year. This training class is approximately one hour in length. After attending the course, the contract supervisor will train the attendants in proper food handling techniques. The contractor will be provided a copy of the roster showing attendance at this training. The training is repeated annually or as often as necessary to ensure attendants are aware of their responsibilities in safe food handlings. 6.Prior to starting work, dinning hall attendants will receive a pre-employment sanitation briefing. Prior to beginning of each shift, contractor's supervisor and the Dining Facility Supervisor will both informally inspect contractor's attendants to ensure they meet the personal hygiene requirements. 7.The contractor shall be responsible for safeguarding all Government property provided for Contractor use. At the end of each work period, all Government facilities, equipment and materials shall be secured. 8.Contract employees must be able to communicate in intelligible English themselves or through the contractors supervisor acting as an interpreter or the contractor must furnish an interpreter. The Government must be able to communicate with the contractors employees due to safety concerns. Normally, the Government will communicate issues to the on-site supervisor. If the Government is unable to communicate with a contractors employee, either directly or through the contractors on-site supervisor, the Government reserves the right to have the contractors employee removed immediately from the job site due to safety concerns. 9.The contractors employees must be either a U.S. citizen or be legally authorized to work in the U.S. Proof of such must be on file for each of the contractors employees. Refer to the Department of Labors website, www.dol.gov. 3.Housekeeping and Sanitation Requirements The Contractor shall maintain standards of housekeeping and sanitation so that dining facilities are in a clean, orderly, and sanitary condition at all times and meet requirements of the Occupational Safety and Health Act and the House Keeping Services Task and Frequency Chart (Exhibit 2). A.Waste Management 1.When garbage cans are filled (not more than four inches from the top) the cans shall be removed to the proper dumpster area and emptied. 2.The trash cans storage, compactors and dumpsters shall be kept in a neat and orderly manner at all times at all times. 3.Outside storage of unprotected plastic or paper bags or baled units containing garbage or refuse is prohibited. Trash and garbage will be bound in such a manner as to eliminate wind blown debris. B.Equipment and Utensil Sanitation 1.Food service equipment shall be cleaned and sanitized to be free of food particles, other soil, grease and cleaning/sanitizing agent residue. 2.All roasting pans, baking pans, steam table inserts and cooking utensils used in preparation and service of food shall be thoroughly cleaned and sanitized after each use. Included in this requirement are racks which military cooks will remove from ovens and range, and components of food service equipment. 3.Pans and utensils will be returned to point of use or storage after cleaning. Food service personnel will transport other machinery components to cook's work area for assembly. 4.Pan and utensil washing will be accomplished early enough as to not cause a delay for cooks requiring this equipment for subsequent food preparation. 5.Tableware, cookware, kitchen and serving line utensils shall be washed, rinsed sanitized and air-dried after each use. They must be visually clean, free of food residue 6.Kitchen exhaust hood, duct, louver and filters (including light fixtures and extinguisher equipment) shall be thoroughly cleaned with a grease cutting cleaner as safety permits. 7.Serving line exhaust hoods shall be damp wiped after each meal. They May be cleaned with a dust mop on an extension handle if safety is an issue. This shall be done before the next meal period. 8.The exterior of the ice machine shall be wiped daily to remove soil, grease C.Handling and Storage of Equipment and Utensils 1.Cleaned and sanitized equipment and utensils shall be handled in a way that protects them from contamination. Spoons, knives, and forks shall be touched only by their handles. Cups, glasses, bowls, plates and related items shall be handled without contacting inside surfaces or the surfaces that contact the user mouth. 2.Cleaned and sanitized utensils and equipment shall be stored at least six inches above the floor in a clean, dry location in a way that protects them from contamination by splash, dust, and other means. Storage racks shall be cleaned with a sanitizing solution each day. 3.Utensils shall be air-dried before being stored or shall be stored in a self-draining position. 4.Glasses, cups, and dinnerware shall be stored inverted. Holders for the storage of knives, forks, and spoons shall be used to present the handle only to the patron. D.Facility Maintenance The Dining Facility including all entrance and exit passageways, hallways, walls, doors, windows and other glass are the responsibility of the Contractor attendants maintain. 1.The clipper room shall be maintained and cleaned as follows: After each meal: a.Sweep and wet mop entire area. b.Damp wipe the exterior surface of the equipment, splashguard, and surface areas. c.Empty and clean trash cans. 2.Dining room shall be maintained and cleaned as follows: During each meal: a.Immediately wipe, damp mop or spot clean as needed any spillage on floor counters, tables or any other surface area as they occur. b.Any spillage within the serving or beverage line area will be immediately wiped or mopped as needed. c.The area around the beverage dispensers, hot and cold lines, and salad bar will receive special attention to ensure the areas are maintained and cleaned, in an orderly fashion. d.Wipe chairs and tables free of crumbs or liquid and reposition chairs around tables as required. e.The patrons of the 256 and 242 will bus their own tables the contractor will not be required to provide bussing service after these patrons finish their meals. After each meal: a.Reposition chairs and tables in preparation for the next meal. b.Clean flatware dispensers and tray racks. c.Clean beverage dispensers, hot and cold lines and salad bar. d.Sweep and wet mop entire area. 3.Dishwashing room shall be maintained and cleaned as follows: After each meal: a.Dining hall attendants will be responsible for cleaning all eating utensils in a sanitary fashion acceptable to the Medical Officer. b.Dishwashing equipment will be cleaned thoroughly at the end of each meal. c.Dish tables will be scrubbed with detergent and rinsed clean with water. d.Floors swept and wet mopped at the end of each meal. 4.Kitchen area shall be maintained and cleaned as follows: Start of Day: a. Set out sanitation buckets as required During each meal: a.Immediately wipe, damp mop or spot clean as needed any spillage on floors, counters, tables or any other surface area as they occur. b.Trash cans will be emptied and relined after each meal. After each meal or as required: c.Damp wipe the exterior surface of the equipment, and the entire serving line d.Empty and clean trash cans. e.Sweep and wet mop entire area. 5.The stock room, dining facility office and refrigerator floors shall be swept and damp mopped daily. Empty garbage daily from the stock room and food service office. 6.All floors that are unfinished, i.e. concrete floors will be swept and then washed.
