Contracting Office Address: Department of the Treasury, Bureau of the Public Debt (BPD), Division of Procurement, 200 Third Street, Avery 5F, Parkersburg WV 26101. Request for Information (RFI) Electronic Employee Satisfaction Survey Services Description: This is a sources sought notice and not a request for quotations. This request for information (RFI) is posted so that BPD can assess the market and determine if small businesses possess the capabilities to provide an electronic employee satisfaction survey and related services as described in this announcement. The objective is to procure general purpose electronic employee satisfaction survey services. Approximately 2,000 employees will receive this survey. This survey will be comprised of approximately 60 multiple choice questions that will be grouped by areas of interest (e.g., work experiences, benefits, demographics, etc.). Five radio button responses ranging from strongly agree to strongly disagree will be used for the majority of the questions. We would like to have at least three (3) text boxes with a minimum character limit of 1,000 each to allow employees to provide narrative responses or further explanation. The survey should include the following: • Privacy Act Statement; • Introductory memo • Instructions for completing the survey, accessing the survey, taking the survey, and help; • Definitions; • Questions (The Bureau of Public Debt will provide) The Bureau of Public Debt (BPD) will need the survey result to be broken out by organizational subcomponents (i.e., the ability to drill data down to major offices). Subcomponents with at least 12 employees will be considered as a major office and will need to be included in the drill down. Approximately 65 subcomponents are anticipated. Data will be provided in an Excel spreadsheet. Contractor will have to provide port/protocol for incoming electronic survey/email prior to the survey. Such information may be included: • Web address • URLs • IP addresses The deliverable for this procurement is an employee satisfaction survey that meets the objective and requirements stated above. The deliverable must also be capable of running on Windows based personal computers. The contractor would be responsible providing the survey to The Bureau of Public Debt, receiving the survey results, and submitting results in Excel format back to BPD. Responses from interested vendors shall include Company Name, socioeconomic category (SDVOSB, HUBZone, SDB, MBE, or small business), DUNS#, address, telephone number, point of contact, email address and brief capabilities statement. In addition, vendors must provide three (3) relevant past performance references that can be contacted and attest to their competencies. The contact information requested shall include at a minimum: name of organization, primary point of contact (name and phone number), brief description of the survey services, and date of performance. If applicable, please provide information regarding current Government contract vehicles that your company been awarded (example: GSA). Please Note: The information being requested is for market research purposes only. The Government does not intend to award a contract based on the information received or otherwise pay for the information requested. BPD will assess the ability of the market to meet the needs as statement above based on the information received. Dependent upon the results of this market survey, BPD may subsequently issue a Request for Proposal (RFP) to acquire electronic employee satisfaction survey services. If you feel that your company possesses the qualifications and an interest to provide this type of services, please send your RFI responses to PSB2@bpd.treas.gov and Angala.Zoller@bpd.treas.gov. RFI-BPD-09-0023 and attention: Angala Zoller must be included in the subject line of your response. Your responses must be no more than six (6) pages (includes any marketing information) and received no later than 2:00 pm ET on March 26, 2009.
