- Presolicitation NoticeThis requirement from The Veteran's Affairs, VISN 20 Contracting Division issued as Request for Quotation (RFQ), number VA-260-09-RQ-0280 with amendments, if any follow, is to provide home lab draws, as specified, in the homes of eligible veterans who are living within 25 miles of the Portland VA Medical Center. Requirements will be driven by referrals generated from the Portland Veterans Affairs Medical Center (PVAMC) Program @Home (home hospital program). This contract will delineate reimbursement rates when home lab draw services are paid directly by PVAMC. The PVAMC will determine eligibility of the beneficiary through the Program @Home Coordinator and attending physician. These include orders of patient admission and discharge to the program. Contract requirements, together with evaluation criteria will be covered in detail in the solicitation and work statement. This requirement is for commercial services in accordance with the format in FAR Subpart 12, as supplemented with additional information as needed. All contractual and technical questions must be in writing (e-mail or fax). Only written requests received directly from the requester are acceptable (Telephone questions will not be accepted). Preference is that you send your correspondence through email at: dennis.riches@va.gov. If requesting by FAX, you may send your request 360-256-0936, ATTN: Dennis Riches. Potential offerors making a written request must include an adequate justification for not obtaining the solicitation via the internet. Telephone requests for diskettes or other inquiries will not be honored. The government intends to award a Firm-Fixed Price (FFP) type contract. It is anticipated that the period of performance will be for one base year, plus four one-year option periods. The RFQ will only be available on the FedBizOpps website at http://www.fedbizopps.gov/. Prospective offerors are responsible for downloading the RFQ, all amendments thereto, and all attachments. It is the offerror's responsibility to monitor FedBizOpps for the release of the solicitation and any amendments that may follow. All quotes must include signed copies of all solicitation documents, including all amendments. Contract award will be based on the best-value to the Government where technical merit and past performance, when combined, are approximately equal to cost or price. The estimated award date is March 31, 2009. This procurement is being solicited on a "full and open/unrestricted" basis. All responsible sources may submit an offer which shall be considered by the agency. The North American Industry Code for this procurement is 621999 for (All Other Miscellaneous Ambulatory Health Care Services), and the size standard is $10 million. Offerors shall identify the size of their business in accordance with this NAICS Code. To be eligible for award, offerors must be registered with the Central Contractor Registry (CCR) at www.ccr.gov. Additionally, Representations and Certifications must be completed at http://orca.bpn.gov. If offerors are not registered via the ORCA website at http://orca.bpn.gov, offerors shall complete and return a copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with their offer. These representations and certifications will be incorporated by reference in any resultant contract. Quote/Proposal must include, Solicitation Number VA-260-09-RQ-0280, Taxpayer Identification Number (TIN), Dun & Bradstreet (DUNS) Number, and all solicitation and proposal documents (including amendments and official proposal documents) must be signed by an authorized company representative.
