Description
This is NOT a request for proposals. The Department of Veterans Affairs, John D. Dingell VA Medical Center, Detroit, Michigan is seeking potential Service Disabled Veteran Owned Small Business and/or Veteran Owned Small Business that have the capability to provide non emergent patient transportation. The minimum requirements for the transportation contract are as follows: Listed are the minimum requirements of this contract: 1.High Top Vehicles 2.Vehicles must be three years old or less 3.Equipped with child safety locks 4.Global Positioning System (GPS) 5.Safety belts and tie downs 6.Minimum five passenger capacity 7.Wheelchair lift 8.Basic first aid kit 9.Fire extinguisher 10.Cardiopulmonary resuscitation equipment 11.Provide quarterly maintenance reports and customer service surveys to the Contracting Officer Technical Representative (COTR) 12.Be able to secure oxygen tanks 13.Quality Management Plan and Customer Service Plan 14.Drug testing information THE FOLLOWING INFORMATION IS ONLY A BRIEF DESCRIPTION OF THE STATEMENT OF WORK. THIS IS FOR INFORMATION PURPOSED ONLY This contract will include transporting patients between veteran's residences or pick up location and VAMC Ann Arbor, Battle Creek, Saginaw and Detroit, Michigan, VA Outpatient Clinics in Toledo, Ohio and two Community Based Outpatient Clinics (CBOC) in Yale and Pontiac, Michigan to and from Chicago, Illinois, Cleveland, Ohio, Iron Mountain, Michigan and Milwaukee, Wisconsin. The estimated quantities are as follows: Wheelchair and or three wheel carts such as Amigos Within 20 mile radius of the VAMC Detroit: 17,088 per year Mileage outside the 20 Mile radius of the VAMC Detroit387,408 per year CBOC Yale 1,200 per year CBOC Pontiac 240 per year Ambulatory Within 20 mile radius of the VAMC Detroit: 13,416 per year Mileage outside the 20 Mile radius of the VAMC Detroit358,992 per year The contact will have a base year plus 4 one year options. The estimated award date of the contract is July 1, 2009. The estimated dollar value of this contract is between $20,000,000 and $50,000,000 The North American Industrial Classification System (NAICS) code is 485999, which has a size standard of $7 M in average annual receipts. The project requires the contractor to be regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish services in the volume required for all the items under this contract. All qualified Service Disabled Veteran Owned Small Business and Veteran Owned Small Business are encouraged to respond. The response to this notice shall be in summary format and shall not exceed three (3) pages. 1. Company name, address, point of contact, telephone number, fax number and email address. 2. List the last three contracts of similar size and scope to include contract number, award date, dollar value of contract, project location and point of contact. No basis for claim against the Government shall arise as a result of a response to this sources sought or Government use of any information provided. The Government will not pay for information or comments provided and will not recognize any costs associated with the submission of comments. All SDVOSB and VOSB firms that are interested in responding are invited to submit their information via email only to the Contracting Officer at Barbara.Wilkinson@va.gov. Attachments must be provided in either Microsoft Work or Adobe Acrobat PDF format. Please send responses by no later the 2:00PM local prevailing time in Detroit, Michigan on March 18, 2009.
