Description
The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas, intends to make a single award for the provision of Physical Therapist Services for the Federal Correctional Institution, located in Morgantown, West Virginia. The contract type is an indefinite delivery/requirements contract with firm-fixed unit prices. The contractor shall provide Physical Therapy services for the inmate population in a correctional setting for the Health Services Department at FCI Morgantown, is available at www.bop.gov. The contract period will consist of a base period from date of award through 12 months, and four one-year option periods for renewal at the unilateral discretion of the government. The estimated quantities for the base year and each option year are 156 sessions per year. A session is defined as one hour. The schedule will be in blocks of three sessions, one day per week. Additional sessions may be provided on an as needed basis. The schedule shall not exceed 30 hours per week. The Contractor will normally work, Monday through Friday (except federal holidays), between the hours of 7:30 a.m. and 4:00 p.m. http://www.bop.gvo. The contractor shall provide physical therapy services in accordance with specific orders provided by the Clinical Director. All modalities needed according to the orders from the Clinical Director will be provide. The contractor will provide education to inmates on physical therapy exercises/modalities used and will only provide physical therapy/modalities to extent where the inmate is able to begin performing exercise on his own with instructions. The Physical Therapist will keep open communication with the Clinical Director and a written report on the progress of each patient needs to be available immediately after each one hour session. The Contractor must be knowledgeable of and comply with JCAHO standards. The Contractor will provide Physical Therapy services in accordance with Bureau of Prison policy. The Contractor is required to be experienced with the delivery of physical therapy services and be able to meet the requirements of this solicitation. Offers are requested to submit photocopies of their resume or license and/or board certification, and sign a consent for release of Information for verification of credentials along with their quote price per session. The solicitation will be made available on or about March 19, 2009, and will be distributed solely through the General Services Administration Federal Business Opportunities website at www.fedbizopps.gov. Hard copies of the solicitation will not be available. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The North American Industrial Classification (NAICS) code is 621340, with a small business size standard of $7.0 million. All contractors doing business with the Federal Government must be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov. The date for receipt of quotations is on/about April 6, 2009; however, this is a projected estimate and quoters should refer to Page 1 of the solicitation (Block #8) for the actual date quotations are due. Faith-Based and Community-Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This solicitation is a total small business set-aside. All responsible small business concerns are encouraged to submit a quotation which may be considered.
