Washington St. Phoenix, AZ. 85003 Kevin Winschel 602-514-7400 Project: Lead Abatement and Filter Abatement of Firing Range The contractor shall provide all necessary material, labor, and supervision for the cleaning and removal and replacement of the firing range exhaust fan pre-filters and HEPA filters. This work is located at the Sandra Day O’Connor Federal Courthouse located at 401 W. Washington St. Phoenix, Az. Proposal to be submitted on company letterhead indicating offer price in US dollars with material and labor break outs, included all applicable overheads, taxes, bonds and profit cost within the material and labor divisions. Government reserves the right to award without negotiations, or request proposal clarifications from any or all offerors. Scope of Work The contractor shall remove and replace all the filters with specified filters or equal. This process shall be performed under “lead” abatement regulations and procedures. The contractor shall provide the Government with all documents and manifests resulting from this work. All work is to be performed in accordance with all applicable codes and trade practices, including, but not limited to, testing, handling and disposal permits. Testing procedure shall comply with HUD standard of 100 or less. Scope of work to include but not limited to the following: Provide all labor, permits, tests for removal and disposal of all material listed below: 1.Provide an initial wipe sample test for exhaust duct/port after filter bank. This test will be used to determine if the filter bank, duct seals are intact and performing to specification. The test samples shall be performed before any of the following work is started. 2.Remove and dispose of all existing “pre-filters”. Replace with new. Photograph the filters prior to removing. 3.Remove and dispose of all existing “HEPA filters”. Replace with new. Replace with new. 4.Clean filter bank before installation of filters to HUD standard of 100 or less. Filter List : (8) Camfil -Farr 30/30: 24” x 24” x 2” - 1000/2000 CFM (8) Camfil -Areopleat IV+: 24” x 24” x 4” - 1000/2000 CFM (8) Camfil – xh absolute HEPA 1801H-0302 See attached product cut sheets for filter specifications and installation. (See Exhibit 1, 10 pages) The contractor shall provide GSA with all manuals, warranty sheets for new equipment installed if applicable. 1.The contractor shall coordinate the project and all involved parties. 2.The contractor shall not interrupt the daily activities of the building tenants. 3.The contractor shall maintain a job file and conduct a final inspection for acceptance. 4.The contractor shall follow up any punch list items to insure compliance. 5.The contractor shall leave the affected areas in a neat and clean fashion during and upon completion of work. 6.The contractor shall submit a construction schedule for GSA or agency approval before starting work. 7.The contractor shall not start work until a written “Notice to Proceed” is issued by GSA. 8.The contractor shall finish work in accordance with contract schedule. 9.GSA will provide partial payments on this contract only for the April cleaning visit and October, 2009 visit. 10. Cleanings shall be scheduled for April, 2009 and October, 2009 plus two optional years. Article I. Security Clearance Prior to start of work the contractor shall submit a background clearance information form for all employees that will require access to government owned space. Contract employees determined to be unsuitable will not be allowed access to the work site. Please submit your DUN# and TIN# with your bid.
