Solicitation: HSFEHQ-09-Q-0048 Description: Diesel Fuel - Louisiana Contract Specialist Armetia Cato FEMA/AMD/Logistics Section 202-646-7978 (w) 202-646-3318 (fax) Contracting Officer: Mary M. Petruzzo FEMA/AMD/Logistics Section 202-646-2603 (w) 202-646-3318 (fax) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. HSFEHQ-09-Q-0048 is issued as a Request for Quotation (RFQ) pursuant to FAR Part 12.6. It is anticipated that one (1) Requirements Contract will result from this synopsis/solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This requirement is a Total Small Business Set-aside and the associated NAICS code is 424720. The small business size standard is 100 employees. Contract Line Item Number (CLIN) 0001 - Base Year - The period of performance is 3/16/09 - 3//15/2010. The Contractor shall provide diesel fuel (3 NAPG High Sulfur - off road only) to FEMA sites located throughout the state of Louisiana. The Contractor shall provide fuel trucks, appropriately licensed drivers/operators, all associated equipment, supplies, fuel, and personnel to handle and dispense hazards materials (diesel). The current delivery addresses are included in this line item; delivery shall be made approximately two (2) times per month on an as needed, on-call basis. Delivery shall be made within 72 hours of request for delivery. The minimum quantity is 6,400 gallons per year; the maximum quantity is 77,000 gallons per year. Current delivery addresses: Unit Price: $ (OPIS plus fixed price mark-up) Location 1: FEMA/THU, 5310 Old State Highway Lottie, LA 70756; 1 tank with 1,000 gallon capacity Monitor - Yes Location 2: FEMA/THU, 365 Old Airport Road DeRidder, LA 70634; 3 tanks with 500 gallon capacity each 2 with monitors/ 1 without monitor Location 3: FEMA/THU, 586 Schexnayder Lane Melville, LA 71353; 2 tanks with 500 gallon capacity each Monitors - Yes Location 4: FEMA/THU, 2695 N. Sherwood Forest Drive Baton Rouge, LA 70814; 1 tank with 1,000 gallon capacity Monitor - Yes CLIN 0002 - Option Year 1- The period of performance is 3/16/2010 - 3/15/2011. The Contractor shall provide fuel trucks, appropriately licensed drivers/operators, all associated equipment, supplies, fuel, and personnel to handle and dispense hazards materials (diesel). The current delivery addresses are included in this line item; delivery shall be made approximately two (2) times per month on an as needed, on-call basis. Unit Price: $ (OPIS plus fixed price mark-up) Ordering instructions: The Government will issue delivery orders which will include the following information: •(1) Date of order •(2) Contract Number and Order Number •(3) Delivery or performance Schedule •(4) Place of delivery or performance THU Site/s (including consignee) •(5) Any delivery instructions •(6) Accounting and appropriation data The Order will provide funding for the estimated number of gallons at the prices established in the contract and the contractor shall bill the Government for only the quantity delivered. The Contractor shall provide proper documentation with each delivery stating the quantity of gallons dispensed/price per gallon as stated in contract/and location/s service shall be left with the consignee. Instructions to Offerors: Offerors shall provide a quotation that includes: 1) The price per gallon of fuel will be based on the OPIS rack price by brand on the specific date the fuel is loaded at the refinery. The Government will pay the current OPIS price at the time of delivery plus the accepted mark-up price. The Offeror shall provide with it's quote the fixed mark-up price per gallon per CLIN (i.e., the quotation should include CLIN 0001, 0002, 0003, to show the fixed mark-up price per gallon). 2) The contractor shall provide three (3) past performance references for similar services to include the point of contact name, phone and contract number under which services were provided. If no past performance exists, the offeror shall state this in the quotation. 3) The cost for renting a monitor for one (1) tank at Location 2. FAR 52.212-2, Evaluation-Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: past performance and price. Past performance is more important than price.
