- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation of Purchase Request #: W68MX390135855 is issued as a request for quotation (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-30 and DAC 20090115. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112. The Size Standard for small business is 500 employees. This RFQ is being issued as Unrestricted. These items are for use at Madigan Army Medical Center, Tacoma, WA. All responsible Contractors can provide an offer for the following Brand Name or Equal Products. Line Item 0001: Hemodialysis System Fresenius 2008K2 System or equal with Bicarb, SMS option, saline drain, buckets, internal blood pressure module and 10cc heparin pump, online clearance, diasafe plus. #: 190371 Line Item 0002: Millenium M-750 RO System w/pressure switch and docking station, #: Z1238205 Line Item 0003: CRRT Eprom #: 190354 Line Item 0004: Acute Wheel Package #: 150150 Line Item 0005: Blood volume monitor #M31909 Delivery shall be FOB Destination, Madigan Army Medical Center. Pricing quoted shall include all applicable fees. The following FAR clauses and provisions in their latest editions apply to this combined synopsis/solicitation. (All clauses throughout this combined synopsis/solicitation can be viewed by accessing the website http://farsite.hill.af.mil/.)
