Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-30, dated January 15, 2009. The North American Industry Classification System (NAICS) number is 811310 and the business size standard is $7 million. REQUESTERS MUST STATE THEIR BUSINESS SIZE ON THEIR QUOTE. This procurement is a total small business set-aside. Offeror's quotes shall be submitted on letterhead and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. The Consolidated Mail Outpatient Pharmacy (CMOP), 2962 S. Longhorn Drive, Lancaster, Texas 75134 has a requirement for the following service(s): Providing all necessary labor, equipment, and material to perform miscellaneous electrical repairs and cabling on an on-call basis (between zero and twenty hours per month) for a Knapp conveyor system and pill dispenser, and a Proveda Plastic Shredder, for a five year period (base and four option years). The contractor shall be available to receive emergency calls during operational hours, 8:00 a.m. through 4:30 p.m. Monday through Friday. The contractor must respond with a phone call within one hour after receiving notification from the CMOP. Contractor must be licensed, bonded and insured. Base Year, Unit/Hourly Rate $__________; Option Year One (1), Unit/Hourly Rate $____________; Option Year Two (2), Unit/Hourly Rate $_________; Option Year Three (3), Unit/Hourly Rate $_______; Option Year Four (4), Unit/Hourly Rate $___________. (Provide both regular rate and overtime rate). The Government will award a contract resulting from this combined synopsis/solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes, in descending order of importance: (i) Technical Capability Provide a statement of work addressing how the offeror plans to accomplish the requirement. The statement of work will contain how the work will be performed. Past performance: provide a minimum of three (3) current references from organizations (Federal, State, local government, or private) under which you have provided the same services required in this solicitation for which you have performed services similar to those required under this contract. Include dates of performance, organization name, address, contact person, telephone number, and reason for termination/cancellation, as applicable. (ii) Price; Technical Capability is more important when compared to Price. Evaluation of past performance will be a subjective assessment based on consideration of all relevant facts and circumstances. In addition to contacting references, the Government may use information from its own files, database sources available for government use such as the Past Performance Information Retrieval System (PPIRS) and Contractor Performance System (CPS), public internet sources, or any other source the Government deems appropriate. The CMOP's evaluation team consists of CMOP employees who have expertise and knowledge of system. The CMOP deems these representatives as experts; therefore, they are qualified to evaluate information considering product quality and consistency and price and other factors to determine which proposal is the "best value" to the Government. The Government intends to evaluate offers and award without discussions with offerors. Therefore, each initial offer should contain the offeror's best terms from a cost or price standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. NOTE: Offerors are urged and expected to inspect the site where the work will be performed. To schedule a site visit, contact Linda Bayer, 913-758-4715 or Linda.Bayer@va.gov. The Government will not pay nor reimburse any costs associated with attending this site visit. A signed copy of the BAA (Business Associate Agreement) must be submitted at the time of or prior to attending the site visit at the CMOP. Wage Determination 05-2509 (Rev 6) is applicable to this solicitation. This wage determination can be accessed at www.wdol.gov/sca. If your company has a Federal Supply Schedule, please indicate the FSS number on your quote. Delivery shall be FOB DESTINATION to the Consolidated Mail Outpatient Pharmacy (CMOP), 2962 S. Longhorn Drive, Lancaster, Texas 75134. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial Items. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, The Government will award a contract resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technically acceptable and (2) Price. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer as applicable. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: (b) 6, 14, 16,17,18,19, 20, 22, 36 and (c) 1 & 2 are considered checked and apply. The Federal Acquisition Regulations (FAR) clauses may be accessed on the Internet at http://www.arnet.gov/far; VAAR clauses may be accessed at http://farsite.hill.af.mil/VFvar1.HTM. The following additional clauses apply: CCR Clause--Attention: You must be registered in CCR to be considered for award. FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide its DUNS or, if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the CCR database. Lack of registration in the CCR database will make an offeror ineligible for award. Offerors and contractors may obtain information on registration and annual confirmation requirements via the Internet at http://www.ccr.gov. FAR 52.252-2, Clauses Incorporated by References (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text: 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 52.228-5 Insurance-Work on a Government Installation; Supplemental Insurance Requirement; 52.235-1 Privacy or Security Safeguards; 52.242-13 Bankruptcy; 52.242-15 Stop Work Order; 852.237-70 Contractor Responsibilities; 852.270-1 Representatives of contracting officers; 852.270-4, Commercial Advertising; 852.273-70 Late Offerors. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, company name, and point of contact, address, phone number, fax number, tax identification number, DUNS number, and size status (i.e., small business, disadvantaged business or woman owned business).
