- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). The purpose of this procurement is to purchase annual accreditation for Aerospace Medicine, Occupational Medicine, and General Preventive Medicine programs for the United States School of Aerospace Medicine (USAFSAM). USAFSAM’s Residency in Aerospace Medicine prepares selected flight surgeons to accomplish all aspects of operational medical support to include the practice of aerospace and occupational medicine, to be authorities in the fields of aerospace medicine and preventive or occupational medicine, and to be board certified as specialists in aerospace medicine and preventive or occupational medicine. The Accreditation Council for Graduate Medical Education (ACGME) is responsible for certifications for the United States Medical Residency program. This is an intention to purchase this system from a sole source (the ACGME). The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-31 (effective 19 March 2009) and Defense Federal Acquisition Regulation Supplement change notice 20090115. FAR 52.212-4 and 52.212-5 applies to this acquisition. WAWF procedures also apply. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. FAR 52.212-2, Evaluation - Commercial Items, applies. Technical capability will be evaluated by how well the proposed product meets the government requirement instead of predetermined sub-factors. Evaluation Basis for Award: Award will be made to the offeror who has the lowest priced, technically acceptable proposal. It is the offeror's responsibility to be familiar with applicable clauses and provisions. The delivery date of these items is 2 weeks after receipt of Order (WARO), FOB Destination, Brooks City-Base, TX. The NAICS Code is 611710 and the size standard is $7M. The type contract anticipated is firm-fixed price. The response date is 12:00 PM, Central Time, 30 Mar 09. The 311th Human Systems Wing/PKAC, Brooks City-Base TX intends to award a purchase order by other than full and open competition in accordance with Subpart 6.302-1 (c). Only one responsible source and no other supplies or services will satisfy agency requirements.
