Definitions 8. Special delivery Summary Contractors Checklist STATEMENT OF WORK DESCRIPTION OF SERVICES 1. Scope of Work. The contractor shall, except as specified in this Statement of Work (SOW) as government furnished property or services shall provide all personnel, supervision, and services necessary to perform Mess Attendant Services at Tulsa Ok, ANGB as defined by this SOW. The contractor shall perform the requirements in this SOW and conform to the professional standards identified in this contract, and should follow all applicable instructions and directives as directed by this SOW. Contractor tasks include, but are not limited to, the following: cleaning of facilities beyond the serving area and all utensils and equipment associated on the serving line (doesn't include steam lines, actual serving area). The contractor also will be responsible for the stocking and replenishing condiments on dining tables. At the present time there is no need for cashier duties, handling of foods or major/minor repair of food service equipment. 2. Program Overview. Aero Space Dining Facility, building 240 at Tulsa ANGB, Tulsa Ok is an Appropriated Funds Dining Facility. The facility is dedicated to providing full meal service and meeting the nutritional needs of the Department of Defense (DOD) personnel who frequent the facility. Aero Space Dining Hall is a full service dining facility which also includes some carry out service and meals to go. The 138th Services Flight kitchen provides boxed meals in support of flying missions and ground support operations upon a normal request period. Our customers are generally Subsistence in Kind (SIK), Active Guard and Reserve (AGR) and Officers of supporting units associated within the 138th FW. SIK personnel are allotted rations in government dining facilities at the expense of the government. AGR and Officers are military personnel who are granted permission to dine in the facility at their own expense. Organizations such as the Boy/Girl Scouts and Civil Air Patrol occasionally dine in the dining facility upon permission from the Base Commander. 3. Operational Requirements and Specific Tasks: A. Pots and Pans. A specific area of concern in the Dining Facility is the availability for food borne illnesses caused by in some instances dirty pots and pans that do not get cleaned accordingly. All pots and pans must be cleaned and sanitized. They must be left to dry and put away in their proper allocated rest area. The contractor will provide knowledgeable person (s) to manage this task for every shift. They will not be responsible for the washing and handling of any knives used in preparation of meals, does not pertain to those knives used as part of normal cutlery used in the consumption of food. B. Replenishment. Prior to each serving the contractor is responsible for initial set up and replenishment of the following: a. Dining table condiments b. Napkins C. Dishwashing. The contractor is responsible for the cleaning and sanitizing in according to the FDA Food Code of all dishware and eating utensils during and after meals. The contractor is also responsible to empty waste barrels during and after meal times. Barrels must be emptied when they become no more than ¾ full. Barrels must be replenished with linings/bags and put back in place. D Sanitation Requirements. The contractor shall comply with all federal, state and U.S. Air Force mandated sanitation requirements. A copy of AFI 48-116, Food Safety Program and all applicable chapters will be provided to the contractor. E. Hood Air Filters. All hood filters will be run through the dishwasher on a quarterly basis to include filters on both Serving Lines and also those over the Food Preparation Area. (January, April, July and October) F. Housekeeping Services: The contractor shall cleanup those areas that immediately affect customer service to include appearance of the Dining Facility. 1. Floor Cleaning. Clean floors so that no visible dirt or dust remains on floors, floor mats or runners, in corners, behind doors, or under furniture. 2. Spot Floor Cleaning. Spot floor cleaning to remove food spills and debris as needed. When cleaning up spots and spills, contractor personnel will use appropriate cleaning solvents/solutions/materials. 3. Lavatories. Spot check lavatories during meal times to ensure tidiness and supplies are available. Clean toilets, wash basins, floors and all wall areas with a germicidal detergent solution prior to, during and after each meal. The cleaning of toilets should include water scale deposits, foreign matter under and on all hard surfaces. 4. Entrance ways and doors. All entrance way doors and flooring will be cleaned monthly to include cashier/head count section. All windows sills will be dusted at the beginning of every meal. All washing/cleaning will be performed with the appropriate solvents/solutions/materials. 5. Other Housekeeping Tasks. a. DINING ROOM: All window sills, chair cushion and legs, table tops and legs, and light fixtures will be cleaned/washed with the appropriate solvents/solutions/materials. b. BATHROOMS and DISHWASHING AREA: All lights, floors, interior walls, sinks and toilets will be washed/cleaned with the appropriate solvent/solutions/materials. G. Waste Management. The contractor shall assist the government by complying with the base recycling program. This may require segregating reusable/recyclable waste materials by type. Non-reusable waste materials (trash and garbage) will be placed in government leased receptacles. Contractor Personnel A. Management. The contractor shall provide a full-time contract manager and/or alternate who will remain on site and be available during the hours of this scope of work. Contract manager and/or alternate will be responsible for ensuring the quality performance (of contractor employees), as they perform within the scope of this statement of work. Contract manager and/or alternate will make themselves readily available to discuss areas of concern within the scope of this statement of work as government personnel identify them. Contract manager and/or alternate will be authorized by the contractor to act on its behalf on all contracts matters relating to daily operation of this contract. Contract manager and/or alternates must be able to read, write, speak, and understand English. This position shall not be vacant at any time. B. Conduct. Contractor employees shall not loiter in any working or patron areas. Upon completion of their assigned shifts, or after eating, employees shall depart from the food service facility only after review of their duties from the 138th Services Flight POC. 4. Hours of Operation A. Normal. Saturday and Sunday operating hours are from 0800-1400. The schedule for FY 10 will be determined in the future. Normally the days service is required is the first weekend of each month. B. Follow on Operations. Contractor will be expected to provide service other than normal operations during Unit Training Assemblies (UTA - one lunch serving on identified monthly Saturday(s) and Sunday(s). The number of operations varies, government will provide contractor with a written 30 day notice of anticipated follow-on requirements. GOVERNMENT-FURNISHED PROPERTY AND SERVICES 1. Property A. Facilities: The Aero Space Dining Hall, Building 240 Tulsa Ok, ANGB is the facility upon which the contract of services is to be completed. B. Equipment Loss or Damage. The contractor shall submit requests to the POC for replacement of government-furnished equipment for processing. Such requests shall specify the reason for the replacement request. 2. Services. The government is responsible for: A. General 1. Removal of any equipment condemned or replaced from the food service facility receiving area. 2. Electrical power into the building, including breaker panel board and circuit breaker 3. All gas, water and steam lines up to and including the supply source for each piece of equipment 4. Repair of all walk-in refrigeration 5. Cleaning, maintenance, and repair of exhaust duct systems through the exterior outlet and exhaust fan system 6. Alteration and repair of building 7. Providing building space to be used by the contractor for inventory and personal storage 8. Supplying of all janitorial supplies, soaps, detergents and equipment necessary to accomplish such tasks. B. Utilities. The government will provide gas, electricity, sewage and water. C. Telephone. "Class C" (on-base) telephone service and access. D. Refuse Collection Point-in the rear of the kitchen is a trash receptacle for food waste and garbage, under normal circumstances a trailer is available for all broken down cardboard boxes. E. Security Forces and Fire Protection is accessed by dialing 911 anywhere on Tulsa ANGB. GENERAL INFORMATION 1. Contractor Personnel Meals. Contractor personnel who work in the 138 FW Dining Facility under this contract are entitled to purchase meals for the local amount for civilians, while on duty and up to 30 minutes prior to meal serving along with the rest of the 138th Services Flight personnel. Meals served to contract personnel will not count toward payment of services. There should not be any snacking of any food served in the facility that is not paid for prior to consumption. 2. Training. The contractor shall provide necessary training to employees and maintain records of all on-site training and have them available for government review. In addition, contractor employees shall receive the government training listed below: 3. Security Requirements. All personnel employed by the contractor in the performance of this contract, or any other representative of the contractor entering government installations, shall abide by all security regulations of that installation. The contractor shall sign an agreement stipulating the security requirements of this contract. A. Base Access. The contractor shall provide to the Dining Facility a list for all employees and vehicles requiring entry on to Tulsa ANGB. The contractor will also be responsible for providing the Dining Facility with changes in a timely manner. B. Physical Security. The contractor shall be responsible for safeguarding all personal items as well as all government property provided. At the end of shift, all government-furnished equipment must be secured. C. Smoking. Those personnel who engage in smoking of tobacco will adhere to all smoking regulations and designated areas. 4. Contractor Furnished Items and Services. Except those items or services already outlined as government-furnished or government reimbursed, the contractor shall furnish the everything required to perform this contract, to include: A. Personnel. The contractor will provide one (1) supervisor/or alternate POC to manage the contract and a minimum of five personnel to meet the requirements set forth by the contract. B. Commercial telephone service. The contractor will furnish their own commercial telephone line or cellular phone service. C. Uniform Requirement. The uniform should consist of clean properly fit black trousers, white shirt, closed to shoes and head gear. 5. Acts of Nature. The contractor shall use reasonable diligence to provide a regular or uninterrupted supply of service, but shall not be liable for damages, breach of contract or otherwise, to the government for failure, suspension, diminution, or other variations of service occasioned by or consequence of any cause beyond the control of the contractor, including but not limited to acts of God or the public enemies, fires, floods, earthquakes, or other catastrophe: strikes; of failure or breakdown of transmission or other facilities. 6. Expected Length of Contract. The 138th Services Flight would like to grant this contract for one year only. DEFINITIONS Basic Allowance for Subsistence (BAS). A cash allowance paid to military personnel in place of subsistence Subsistence. Food Items, including beverages and condiments Subsistence-in-kind (SIK). Food furnished to enlisted personnel at Government expense; instead of a monetary allowance. Food Service Manager. An officer, or non-commissioned officer, responsible to the Chief of Base Services for matters relating to food service; he/she advises the contracting officer on matters relating to the contract Equipment. Items used to store, prepare, cook, transport and serve food. Equipment also includes items used in cleaning and sanitizing, as well as those used to transport and store supplies.
