- Offerors Representation and Certifications Wage Determination Statement of Work General Provisions Special Provisions 27 March 2009 MEMORANDUM FOR Prospective Offeror FROM: 436 CONS/LGCB 639 Atlantic Street Dover AFB DE 19902-5639 1. We are requesting quotes from anyone interested in providing a concessionaire pizza/food service operation at Dover AFB, Delaware. Contract start date is 30 days from date of award for a one-year period with four option years for a total of five years. 2. If you wish to compete for the opportunity to provide this service, please submit a package not later than 15 April 2009, 4:00 PM Eastern Time. 3. United States Signature Brand restaurants are not authorized due to the conflict with Air Force branding. Signature Brand restaurants are name brands such as Applebee's, Pizza Hut, TGIF, Outback. 4. All quotes must be sent via email to luz.santiago@dover.af.mil or faxed to 302 677-2309 ATTN: Luz Santiago, 436 Contracting Squadron, Bldg 639, Room 242, Dover Air Force Base, Delaware 19902-5639. 5. Your package must contain the following. All items must be completed and provided to be considered fully responsive and eligible for award. a. A Cover sheet with your name, company name, address, phone number, fax, & Email. b. Completed Schedule (Article I-IV) filling in the blanks on pages 1 and 2. c. List of items with corresponding selling prices for base year and each option year. d. Offeror Representation and Certifications (Attachment 4) pages 1-16, fill in blanks and applicable blocks. 6. All pages must be completed to be considered responsive. 7. There will be a pre-proposal meeting on 8 April 2009 at 10:00 AM Eastern Time in the Collocated Club (Landings), Dover Air Force Base. The Landings is where the pizza /food service operation will be set up.   8. Security Forces at the Main Gate will be advised of the scheduled meeting in order to allow attending contractors access through the Main Gate on set date and time.
