Description
FISC Norfolk Contracting Department, Philadelphia Office intends to acquire Mobile Lithotripsy services to include Lithotripsy equipment related supplies and support personnel in support of Naval Hospital Pensacola, FL on a 100% Total Small Business Set Aside Basis. The resultant contractor shall provide a Mobile Lithotripsy System and x-ray technician personnel to perform lithotripsy procedures for treatment of kidney stones one (1) weekday per month (e.g., Monday, Tuesday, Wednesday, Thursday, or Friday) at Naval Hospital (NH) Pensacola. Services will not be required on observance of a Federal holiday. The Government will coordinate a delivery schedule with the contractor one month in advance for each 1-month period. Naval Hospital Pensacola anticipates approximately 5 procedures per month. The mobile lithotripsy equipment shall contain the following minimum specifications: Mobile unit, spark-gap technology with 22-24 KV range that is FDA approved for both renal/u! rethral stones, C-arm fluoroscopy and digital imaging, minimum 30 KW generator, and 4-way position-able treatment table. The contractor shall be responsible for all maintenance or repairs necessary to the equipment, and shall provide all necessary software, licenses, and software upgrades for usage with the equipment. Upon each scheduled visit, the Contractor shall deliver, set-up, shut-down, and remove the Lithotripsy System and shall complete the manufacturer routine quality control procedures for the unit. The Government will schedule a Biomed inspection to the performed prior to every scheduled visit. This procurement is being conducted as a commercial item acquisition in accordance with FAR Part 12, using Subpart 13.5 test program procedures. Award of a Firm Fixed Price Purchase Order is anticipated. The proposed action will consist of a one-year base period commencing on or around 01 July 2009 through 30 June 2010, plus four, one-year option provisions to extend the term of the contract through 30 June 2014, if all options are exercised. Technical and price proposals will be required. Upon its issuance, Request for Quotation N00189-09-T-Z055 will be made available electronically at the following internet address: www.neco.navy.mil. Interested parties are responsible for monitoring the abovementioned website for posting of the solicitation and any amendments issued thereto. The applicable North American Industry Classification System (NAICS) Code and size standard for this acquisition are 621999 and $9 Million, respectively.
