This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-09-T-0170 is issued as a request for quotation (RFQ). STATEMENT OF WORK - NON-PERSONAL ROMAN CATHOLIC PRIEST SERVICES 1.SCOPE OF CONTRACT a.The Senior Roman Catholic Chaplain at NMCSD is the official representative of the Archbishop of the Military Services and by virtue of his office is the authorized person to determine the suitability and selection of the Roman Catholic Contract Priest. b.The Senior Roman Catholic Chaplain conjointly with one other NMCSD staff chaplain will conduct and interview each applicant. The interview will reflect a review of the overall package with emphasis on the following: a.Overall professional training and experience of the candidate as a hospital chaplain.b.Cost of the contract.c.Military experience and /or understanding of military methods by the candidate.d.Flexibility of the candidate in meeting the needs of the hospital patients and staff within the parameters of the contract.e.Perceived ability of the candidate to do sustained and intense hospital ministry.f.Perception of the candidate as a team player who can edify and be edified by the Pastoral Care Service Team.g.Overall evaluation of package and all of the above. c.The work to be performed is located within the Pastoral Care Service, Naval Medical Center, San Diego (NMCSD). An ordained Roman Catholic priest shall provide Roman Catholic on-call and scheduled non-personal services d.During the term of this contract, the contractor agrees to perform on behalf of the U.S. Government, the duties of a Roman Catholic priest for the spiritual care of active duty military personnel, their dependents, eligible Navy civilian employees, and other eligible beneficiaries, per the terms and conditions of the contract. The contractor, although in fact not a Government employee, shall comply with Executive Order 11222 of May 8, 1965, "Prescribing Standards of Ethical Conduct for Government Officers and Employees", and shall also comply with Department of Defense (DOD) and Department of Navy (DON) regulations implementing this Executive Order. 2.CONTRACTOR PERFORMANCE a.The contractor shall be responsible for: (1)CLIN 0001: Catholic Priest On-Call Services (using a duty beeper or available phone number) from 1600-0700 on week nights, and/or 0700-0700 (24hrs) Saturdays, Sundays and holidays. On-call services include the sacraments, visitation, and counseling of Catholic patient/personnel, their dependents and other beneficiaries. Circumstances, which require the presence of the contractor, are to be determined by the Contracting Officer's Representative (COR). The contractor shall respond within one (1) hour from the time of call. For these services, the contractor shall be paid a flat fee per month. b.The contractor shall provide a competent priest substitute with equivalent credentials who will do Catholic coverage during periods of contractor absences. c.The contractor shall perform, within the parameters of his ordination and the dictates of his faith group, a full range of pastoral services on-site. The Government will provide a loaner beeper unit which the contractor shall use in the performance of pastoral services and return upon contract performance completion. d.On-call service as scheduled by the DPCS includes visiting any Roman Catholic patient/personnel who request the presence of a priest directly or through family or staff, the normal process of notification as a result of being placed on Serious/Very Serious List, and on the occasion of death. The contractor shall refer patient/personnel to Pastoral Care Service for consultative opinions and continuation of pastoral care, following the usual norms of privilege/confidentiality. 3.PERSONNEL QUALIFICATION REQUIREMENTS a.Specific skills, knowledge, and documentation required of Roman Catholic Priest providers by this contract include: (1)Ecclesiastical endorsement in writing by the Diocesan Bishop or Religious Superior. (2)Ecclesiastical endorsement in writing by the Archdiocese of the Military Service, Post Office Box 4469, Washington DC 20017-0469 sent to the Chief of Chaplains (OP-972) for verification. (3)A physical examination certification which states the date on which the physical examination was conducted, the name of the doctor who performed the examination, and a statement concerning the physical health of the contractor. The certification must contain the following statement: "(Name of Contractor) is suffering from no physical disability or medical condition which would restrict or preclude him/her from providing services as a Roman Catholic Priest. (Name of Contractor) is suffering from no contagious diseases to include but not limited to AIDS, tuberculosis, hepatitis, and venereal disease, and is not positive for HIV antibodies". b.No alien shall be allowed to perform under this contract without an Alien Registration Card ("green card") and/or in violation of the Immigration Laws of the United States; and c.The contractor shall be neat, clean, well groomed, and in appropriate clergy clothing (shirt with Roman collar) when in patient care and public areas. The contractor shall display an identification badge on the right breast of his/her clothing that includes the contractor's full name and professional status. d.The contractor shall become acquainted with and obey all station regulations, shall perform in a manner to preclude the waste of utilities and shall not use Government telephones (or other Government equipment, e.g., loaner beeper unit issued in conjunction with the contract) for personal business. All motor vehicles operated on this installation by the contractor shall be registered with base security service according to applicable directives. Eating and smoking by the contractor are prohibited in patient care areas and are restricted to designated areas. e.The contractor shall be able to read, write, speak and understand English language fluently. -End of Statement of Work- SUBSTITUTION OF PERSONNEL - PASTORAL CARE SERVICE 1.The contractor agrees to assign to the contract only those individuals whose resumes, past performance records, etc. were submitted as required by this contract. 2.The contractor agrees that during the contract performance period no personnel substitutions will be permitted, unless an individual's sudden illness or other incapacitation, as outlined in the Statement of Work (SOW) necessitates such substitutions. 3.All proposed substitutions shall be submitted in writing to the Contracting Officer's Representative (COR) at least within 24 hours prior to the proposed substitution or as soon as the prime contractor's absence becomes imminent. All proposed substitutions shall satisfy the Personnel Qualification Requirements in the SOW. 4.The requesting code shall approve or disapprove any proposed substitutions at any time during the contract performance period. The requesting code shall evaluate and promptly notify the prime contractor, in writing, of whether the proposed substitution is approved or disapproved. 5.If the Contracting Officer determines that suitable and timely replacement of personnel, whom have been reassigned, terminated, or have otherwise become unavailable to perform under the contract, are not reasonably forthcoming, or that the resultant completion of the contract is jeopardized, the contract may be terminated by the Contracting Officer for default or for the convenience of the Government, as appropriate. (End of Text) ADMINISTRATIVE INFORMATION - PASTORAL CARE SERVICES1.Required services shall be scheduled by the requesting code IAW the Statement of Work (SOW). The contractor agrees to comply with the established schedules and coordinate substitutions whenever necessary. 2.To expedite the ecclesiastical endorsement verification process, the Government shall provide, upon contractor request, fax transmission assistance. Arrangements can be made with the Purchasing Agent (block 6) prior to contract performance.3.A Government-furnished beeper unit shall be in the contractor's custody and for which the Contracting Officer's Representative shall explain the contractor (COR) prior to contract performance. 4.The contractor is required to submit to the Government a proper invoice in arrears on a monthly basis. For prompt payment certification purposes, this monthly invoice must itemize the actual services rendered for the month and submitted within the first five (5) working days of the following month.
